CI Mid-Term — Rachel Small Chase always maintains a safe environment for himself and his patient. He is aware of COVID protocols
and precautions. He always wears a mask and cleans each piece of equipment after used by the patient.
Chase can continue to work on improving awareness of 6 ft distances between pts and go to different
equipment or move the equipment to maintain 6 ft distance.

correcting form. He also regularly seeks feedback during manual therapy to ensure the pt's comfort. He is
able to adjust his style of communication depending on the target audience. Over the next 7 weeks I
would like him to focus on communication among PT techs and other clinicians about pt care.

CI Final — Rachel Small Chase communicates in a professional manner with all interactions. He has improved in consistency with
communication with other treating therapists about the patient and patient care. He has also improved
with communication with support staff for things like scheduling as well as getting patient started if
running over with the previous patient.

7. Patient Management – Clinical Reasoning


CI Mid-Term — Rachel Small Chase is progressing with his clinical reasoning on simple cases where he has been treating the patient
and is able to recognize when an exercise is ready to be progressed or if something in the plan isn't
working. He continues to require assistance on if a modality should be utilized and with complex patients
on where to begin their exercises including lower level mat exercises. Over the next several weeks, I
want Chase to continue to progress his clinical reasoning including taking subjective information during
the evaluation and determine 2-3 possible diagnoses and chose testing to to rule in or out those
diagnoses.

CI Final — Rachel Small Chase continues to improve and develop his clinical reasoning. He is confident and independent on
simple cases including new evaluations. He is now able to decide if a modality or intervention should be
progressed or discharged. Chase is also able to determine when a patient is ready to be discharged.
During evaluations, he brainstorms several working diagnoses and is able to chose which special testing
to use to rule in or out those diagnoses. Chase is now able to manage complex patients independently as
well.

CI Mid-Term — Rachel Small Chase utilizes functional outcomes with each patient. He is able to perform an evaluation on a simple
patient. Chase is independent with tests and measures his ROM and strength measurements are accurate.
He does still require assistance choosing tests and measures for more complex cases.

CI Mid-Term — Rachel Small Chase is independent on taking a patient's history and obtaining all the pertinent information. He is
independent in taking that information and determining appropriate tests and measurements on simple
cases but requires assistance on complex cases especially special testing. He needs to continue to work
on sequencing of the tests and measures to limit the patient position changes.

CI Final — Rachel Small Chase is independent on taking a patient's history and obtaining all the pertinent information. He is

cases but requires assistance on complex cases especially special testing. He has improved on sequencing
of tests and measures to reduce patient position changes. He also has improved managing patients who
over share during initial evaluations and has been able to get them on task and get the eval done in a
timely manner. Chase has really improved in establishing several working diagnoses and determine from
the subjective and objective measures the differential diagnosis. Chase is also able to give the patient an
initial HEP that addresses the tests and measures. Chase is able to independently establish and explain to
the patient an initial prognosis and plan.

choosing lower level starting exercises for complex patient cases.

first day treatment including manual and exercise interventions for all initial patients.

leaving a unit on the table and make sure if doing 20 minutes of manual, would the patient benefit from
23 minutes and gain the extra unit. Don't work for free!

CI Final — Rachel Small Chase has improved on gaining the extra unit and even keeping patients longer than 60 minutes when it
would benefit them. He utilizes the rehab tech to start his next patient when he is keeping patients over
the hour.

with 1-2 evals.
