4. Professional Practice – Communication
CI Mid-Term — Rachel Small Chase communicates well with pts. He is excellent at explaining and demonstrating exercises as well as
correcting form. He also regularly seeks feedback during manual therapy to ensure the pt's comfort. He is
able to adjust his style of communication depending on the target audience. Over the next 7 weeks I
would like him to focus on communication among PT techs and other clinicians about pt care.
CI Final — Rachel Small Chase communicates in a professional manner with all interactions. He has improved in consistency with
communication with other treating therapists about the patient and patient care. He has also improved
with communication with support staff for things like scheduling as well as getting patient started if
running over with the previous patient.

CI Final — Rachel Small Chase continues to improve and develop his clinical reasoning. He is confident and independent on
simple cases including new evaluations. He is now able to decide if a modality or intervention should be
progressed or discharged. Chase is also able to determine when a patient is ready to be discharged.
During evaluations, he brainstorms several working diagnoses and is able to chose which special testing
to use to rule in or out those diagnoses. Chase is now able to manage complex patients independently as
well.

independent in taking that information and determining appropriate tests and measurements on simple
cases but requires assistance on complex cases especially special testing. He has improved on sequencing
of tests and measures to reduce patient position changes. He also has improved managing patients who
over share during initial evaluations and has been able to get them on task and get the eval done in a
timely manner. Chase has really improved in establishing several working diagnoses and determine from
the subjective and objective measures the differential diagnosis. Chase is also able to give the patient an
initial HEP that addresses the tests and measures. Chase is able to independently establish and explain to
the patient an initial prognosis and plan.

portion of evaluations, progress notes, and discharge summaries including complex cases.

would benefit them. He utilizes the rehab tech to start his next patient when he is keeping patients over
the hour.

CI Mid-Term — Rachel Small Chase is seeing around 75% of a full case load. Full case load: double booked twice in an 8 hour day
with 1-2 evals.
CI Final — Rachel Small Chase is seeing a full case load including 1-2 evals and a double booking in a day.
