Instructions:

1. All the particulars required must be legibly and accurately filled in by the
applicant in his/her own handwriting.

2. This application form, complete in all respects, duly accompanied by necessary


payment – in shape of a bank draft, pay order or security deposit receipt/ online
Deposit Receipt – in favor of the Treasurer, University of Engineering and
Technology Taxila.

3. Payment can also be made through Bank Challan at the Habib Bank Ltd. UET
Taxila Branch (Attach counterfoil-1 with the application). Payment through any
other mode including cheque or cash is NOT acceptable.

4. Attach the exact number of clear, complete and legible photocopies of the
Credentials which should be returned to the applicant, or posted where so
requested, after verification.

5. Attach the envelope of appropriate size with Postal Stamps of sufficient amount
for Registered Post affixed /Return Envelop of Courier Company with the clear
Address, where the verified copies are to be sent, must be written clearly and
legibly. This office shall not be responsible for any delayed or wrong delivery
thereof. In case No return envelope/stamps are there it will be returned through
Pakistan Post.

6. The current rates of Local verification fee (subject to revision at any time
without any prior notice) are given below:

 Detailed Marks Certificate (DMC) Rs. 250/- per copy per Term
 Semester Grade Sheet Rs. 250/- per copy per Semester
 Provisional Certificate Rs. 250/- per copy
 Transcript of Awards / (TMK) Rs. 500/- per copy
 Degree Rs. 500/- per copy
 Any other Certificate Rs. 250/- per copy

7. All above mentioned verifications are dealt by Controller of Examinations for
