DECLARATION FORM FOR TOURIST TRAVEL MOVEMENT BETWEEN


TOURIST FACILITIES

The below tourist facility agrees to engage in tourist travel movements

In strict adherence to the “Public Health Interventions to prevent COVID-19 Transmission in the
Tourism Sector” and below conditions:

1) Maintain the records of tourist temperature and other COVID-19 related symptoms prior to
departure from the tourist facility as per the ANNEX 5 of “Public Health Interventions to prevent
COVID-19 Transmission in the Tourism Sector” and emailed to [email protected]
prior to any movement.
2) No tourist should leave the tourist facility on a split stay while in isolation, quarantine or if
he/she has fever or develop any COVID-19 related symptoms.
3) No movement should undertake if the tourist facility is declared as “ongoing contact tracing” or
“community spread” status by Health Protection Agency.
4) The tourist facility must make an alternative arrangement for the tourist if the status of the tourist
facility changes while the tourists are on the way to the facility on a split stay.

We hereby declare that all the information provided in the documents is true and correct; and
agrees to fulfill ALL terms and conditions set forth for split stay of tourists.

We are aware that any violation of the terms and conditions pertaining to tourist split stays may
lead to immediate suspension of split stay movement to and from the respective tourist facility;
and temporary suspension of the approval letter “Approval to re-start tourism in Maldives”
issued.
