The nursing care plan addresses a patient with activity intolerance related to low blood pressure and generalized weakness. The plan includes monitoring vital signs hourly, spacing and adjusting activities based on tolerance, elevating the head of the bed, gradually increasing passive range of motion exercises, encouraging rest periods, and assisting with administering bronchodilators. The evaluations found the patient was tolerating interventions and reporting increased activity tolerance and energy levels.
