DIALOGUE PHYSICAL ASSESSMENT

Nopita Sari, 25 years old. While being treated in the Apple room of RS X
with difficulty breathing, a nurse named Widya came to see him with the aim of
doing a physical examination.

TAHAP ORIENTASI
Nurse : “Good morning, Is this true with Mrs. Nopita?”.
Patient : “Good morning, yes true nurse”.
Nurse : “Mrs. Nopita, my name is nurse Widya, I am the guard nurse this
morning. How did Mrs. Nopita sleep yesterday, was it soundly??”.
Patient : “Not very good, nurse, because I still feel short of breath”.
Nurse : “Did Mrs. Nopita have breakfast today?”.
Patient : “Yes nurse, I got my breakfast”.
Nurse : “Yes Mrs. Here I will do a physical examination on you and measure
your vital signs, is Mrs. Nopita willing?
Patient : “Yes, nurse”.
Nurse : “Before I started the examination, was there anything Mrs. Nopita
wanted to ask?”.
Patient : “No, nurse”

TAHAP KERJA
Nurse : “Previously, I will adjust the position of the mother's body first, excuse
me Mrs”.
Patient: “Yes, please nurse”
Nurse : “For example, when I do an examination, you feel pain and others, you
can tell me”.
Patient : “Yes, nurse”
Nurse : “Okay then, for the first. I wiil measure your vital sign, I will take this
thermometer in your armpit, please clip this, and I will fold your sleeve to
easy I take the manset of sphygmomanometer in your arm , and I will
check now”.
Patient : “How is the result nurse?”.
Nurse : “Well, the result of your vital sign are your blood pressure is 120/80
mmHg, your pulse 82 bmp, and your temperature 36,8 degrees celcius”.
Patient : “Okay nurse”.
Nurse : “Now I will continued to do physical esamination to you”
Patient : “Yes, please nurse”.
Nurse : “For the first one, I will check the head of the Mrs first, excuse me mom,
do you feel pain when I touch it and I press a little?”.
Patient : “No, nurse”.
Nurse : “Okay, now I continue to examine the face starting from the eyes, nose,
mouth, ears of Mrs.
Patient : “Yes, Please nurse”
Nurse : “Your lips mucosa look pale and dry, I suggest you drink a lot of water,
because is also important for you and for fluid balance your body”.
Patient : “Thank you for your suggest nurse”.
Nurse : “Now I continue to examine the neck, do you feel pain when I press your
neck?”
Patient : “No, nurse”.
Nurse : “Okay Mrs. Nopita, here I will examine the shoulders, elbows and
hands”
Patient : “Yes, nurse”
Nurse : “Next I will examine the Mrs chest and stomach” (began to perform
examination procedures in the chest which consisted of inspection,
palpation, percussion and auscultation).
Patient : “Yes, Please nurse”.
Nurse : “Mrs. Nopita, try now to inhale and exhale slowly”.
Patient : (inhale and exhale slowly)
Nurse : “Now I will palpate your stomach, do you feel pain?”.
Patient : “No, nurse”
Nurse : “Okay Mrs. Nopita, here I will examine the hips, knees, ankles and soles
of the feet”.
Patient : “Yes, please nurse”.
Nurse : “Can you move the ankle, Mrs?”.
Patient : “Can, nurse”.

TAHAP TERMINASI
Nurse : “Okay Mrs, I have finished doing the examination, how do you feel
now?”.
Patient : “That's better, nurse. But earlier during the examination I felt a little
pain”.
Nurse : “Fine Mrs, thank you for being able to follow my instructions well,
before I leave the room are there any other complaints or want to ask?”.
Patient : “No, nurse”.
Nurse : “Good if later you have complaints, you can press the bell or you can ask
the family to call the nurse in the nurse's room, later around 12.00 I will
come here again to give you medicine”
Patient: “Yes nurse”.
Nurse : “Then you can rest again, I hope you get well soon, I'll excuse myself
from the room, good morning”.
Patient : “Yes, thank you nurse”.
Nurse : “Yes, you're welcome Mrs”.
