    AFFIDAVIT OF DENIAL
    I, ELMOR A. DIAZ of legal age, Filipino, married and a resident of
    San Vicente Bacon District, Sorsogon City after having been sworn to in
    accordance with law do hereby depose and state:

    1. That on March 3, 2020, I received a letter from certain ATTY.


    ROBERTO B. ULTADO JR., demanding in that letter payment to
    Globe Telecom. Inc., amounting to Eighteen Thousand Six
    Hundred Forty-Nine Pesos and Seventy-Six Cents (Php
    18,649.76).
    2. That allegedly according to the letter, the said amount is my unpaid
    balance for an account in the said Telecom;
    3. That I was shocked and alarmed by this since I do not have
    any accounts or whatsoever connected to Globe Telecom;
    4. I don’t know how my name appeared in the said account, but
    definitely I am not the one who was named therein;
    5. That I learned from Globe upon investigation that the owner of the
    said account, who falsely represented me, claimed that I worked in
    Bicol University where in fact I am a farmer. I also learned that it
    was stated therein that my mother’s name is “Asuncion” when the
    real name of my mother is “Lydia.” I attached hereto my birth
    certificate for reference;
    6. That with this I demand an investigation for I cannot be held liable
    for an obligation that I was not the one who incurred, much more
    for someone who fraudulently used my personal details;
    7. That I am executing this affidavit in order to attest to the veracity
    of the foregoing statements and to affirm that the same are true and
    correct to the best of my knowledge and belief, particularly to
    deny liability to Globe Telecom, that I am NOT the named
    ELMER ANDULAN DIAZ that has an account with them, and
    for all legal intents and purposes this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 3rd day


    of March, 2020 at Sorsogon City.
