making sure that all legal requirements are satisfied. Also, taking action to have inspections. Ans all

In the area of WHS risk management, define the hierarchy of control and describe how you would use

It is a system that can help control risks at the workplace. By using step-by-step measurement, it help
eliminate risks and also ranks risks from highest level to the lowest.
If I need to use this measurement, I’d like to eliminate hazards and risks. This is at the highest level of
the protection and control. By doing this, it is this most effective measurement. After that, I’d like to
reduce the risk. After this, I’d like to have administrative controls. The last but not the least, I’d like to
have personal protective equipment. This is the lowest level of protection and control.

The reason that collaborative and consultative approach plays a significant part in managing
occupational health and safety is that a safe workplace can be more easily built by every person gets
involved in the workplace. By communicating with each other effectively and closely, this can identify
hazards and risks easily. Also, by working together, solutions can be more easily found.

How would you suggest that organisations who are not required to establish a health and safety
committee, set about?
a) Providing information to work teams on identified hazards and risks.
—It is necessary for employees at the team to understand what they might experience in their daily
job. And how they can avoid these from happening. Reference toolbox talks, memos, safety
instructions and SOP's, orientation and other forms of training, notice boards, posters, signage or
other appropriate methods.
b) Recording and addressing safety issues raised by staff.
—This can help managers and business owners to respond appropriately to the concern. By
recording and addressing these issues, a better safety workplace can be provided. Minutes of
meetings, safety records and logs, event and accident forms, surveys and questionnaires, or similar.
c) Communicating to staff members the outcomes of issues raised.
— It is important that this can provide feedback and can build a good working environment. Also,
this can help engage staff and encourage workers to find out any concerns they have. Through
written or verbal communication, either face to face or via electronic formats.

Question 11
Describe at least two instances where you have personally delivered training or coaching to colleagues
or team members in the area of occupational health and safety. In your description please include:
a) The training/coaching need and how it was identified
b) The approach taken to deliver the training/coaching
c) The outcome.
1. How to remove the heavy box. It was identified that a colleague did not correctly bend her back to lift
a heavy box. The coaching was delivered by introducing the correct moving position at the workplace.
The colleague mastered the correct body position to bend the back when lifting heavy items.
2. Wet floor at the workplace. It was identified that the floor was slippery. The approach was to
introduce the management of wet floors. The result was that every time when there is a wet floor, a
signage is always used to prevent employees to be harmed from the wet floor.

Question 12
a) When should incident and accident forms be completed, and how do these records contribute to the
identification, and possible prevention of workplace incidents and accidents?
Within 48 hours. This information can help to identify problems and avoid the repeat of the
incident. Also, they can act as a tool to investigate and assess risks, then eliminate the risk. Also, it
can help to find out solutions to potential risks.

Record about equipment such as inspection or repair.

Question 13
Consider the need for a fire and emergency evacuation procedure in a workplace. Identify the key
content you believe should be included.
Firstly, I need to identify the fire hazards. Secondly, I need to assess the risk. Thirdly, I need to control
the risks by eliminating and reducing the risk. In this part, I need to consider the design, substitution,
engineering and administrative measures. After that, I need to draw a fire and emergency evacuation
procedure.
Also need training and practice drills in the daily workplace.

Question 14
Explain how you keep up to date with changes to WHS legislation, WHS policies and procedures and
other WHS matters.
There are four ways to keep up to date with changes to WHS legislations. Firstly, performing job hazard
analyses. By knowing this risk at the job can minimise the hazard and help to develop measurement to
minimise the risks. Secondly, the business owners can have WHS training regularly. Thirdly, view the
WHS websites (e.g. Worksafe Victoria), keeping up to date with current affairs. Also, the business
owners can attend forums.

Question 15
Sometimes it is not possible to eliminate all risk. What action should be taken to report residual risk?
At the beginning, it is important to identify the relevant governance, risk and compliance requirements.
After that, to check the organisational strengths and weaknesses. At the same time, the exiting problems
need to be acknowledged. Also, needing to know about the risk appetite. The last but not the least,
finding out the choices to offset the unnecessary residual risks.

1. Identify at least four different areas of safety training you believe the organisation in
question should have provided to the worker.

Both first aid training and safe use of machinery for technology can be delivered by attending
face to face course at institutes.

1. The manager does not identify those who are at the most risk and identify to them what the
risky factors facing with them. This can eliminate risks. 2. Also, the manager does not check
the safety guards at the workplace regularly. If he did this regularly, then the accident can be
prevented and reduced. 3. The workplace does not install the engineering controls system. 4.
Inadequate administrative controls was presented at the workplace. There is no practice
guidelines or trainings to help employees to use the machines.
4. What procedures do you believe the organisation should have implemented to control the
risks that led to the incident? You are required to identify at least three. Use the hierarchy
of controls as a basis for your response.
• Eliminating the risk. When the previous incident happened, the business owner should
eliminate the risk. This means that when the first incident related to that machine not fitted
with appropriate safety guards, this risk should be removed.
• Isolating the risk. This can be done by placing the dangerous machine in other room from the
public area and avoid workers from using it.
• Engineering controls. This can be done by installed another safety equipment at the
workplace such as installing safety guards.

5. What incident and accident reporting functionality should have been in place within the
organisation? Could analysis of the aggregate data from these reports potentially have
prevented this injury?
After the previous incident and accident, there should be a report record the accident at work. It
not only includes the details of the incident, but also contains the information about the
investigation. Once the investigation is done, the cause of the accident or incident will be found
out. And changes will be made to reduce and eliminate the same incident from happening.
Yes, I agree that the analysis of the aggregate date for these reports could have potential
prevent this injury.

6. Imagine you were a production worker at Conroy’s and were the only witness to this

In my opinion, Mineral Resources (Sustainable Development) (Mineral Industries) Regulations
2019, Code of practice for mineral exploration and previous Incident and Accident reporting at
mine should be placed within the company. Also, incident report should have been completed
when pre-start check identified that brake warning light was not working and if this occurred on a
number of occasions, management could look into the maintenance requirements to find out
whether the maintenance schedule needed to be changed. By analysing these reports, safety
measures could be maintained, and safety awareness could be developed. Therefore, the
potentially injury could be prevented.

• How will the importance of effective consultation and participative arrangements be communicated

Consolation is very important in order to build a safe and health working environment. Also, this can help
minimise injury at the workplace. Team members should be encouraged to contribute while having
meetings. Also, sending emails, staff meeting, and new employee induction can provide the better
communication approach. Last but not least, if possible, making contact with team members every day.
The message will include everyone’s responsibility to be involved in the consultative WHS process; the
major requirements for consultation; how everyone’s opinion will be listened to.

• What procedures and guidelines should be in place to ensure effective consultation and participation

Thanks for all your participation in our safe consultation meeting.
During the meeting, I presented a lot of information about WHS. The most important one is the one area
identified in our aged care facilities. It is very likely that our aged care workers would hurt themselves
while delivering service to our clients. In order to reduce this risk, as we discussed, we need to deliver
onset to workers training about the safety work procedure and providing protection device and teach
workers how to use safely. At the same time, supervisors and managers are responsible for deliver the
risk management. In terms of follow up worker, this whole implementation will be reviewed in
24/09/2020.
