We are proud to offer to you our comprehensive health, dental, life, and disability benefits. You are eligible to
elect your benefits so long as you have worked for us for at least 60 days, and your coverage will kick in on the
1stof the month after you make your elections. 

Your benefits will be effective December 1, 2019 

You will enroll in our plans via our online system, The BerniePortal. You will receive log in instructions via the
personal email address we have on file for you. If you would like to use a different email, let us know. Once you
log in, you will be guided to learn more about each benefit we offer and then given the option to enroll or waive.
You can enroll in some, all, or none of our plans. If you do not wish to enroll in any plan, please still log in and
select “waive” for all.

 Medical: Cigna with Clarity HRA (Health Reimbursement Arrangement)

 Short-Term Disability: Guardian

 Dependent Care Spending Account: Clarity Benefits

The company covers a share of medical and dental premiums and 100% of vision premiums. The other benefits are
voluntary payroll contributions. Below is information on your share of the cost of medical and dental coverage, which will
be taken weekly as a deduction from your pay.

*If you miss time from work, we will make up the missed premiums on your next paycheck. Your coverage is contingent
on maintaining full-time status with an average of 30 hours worked per week. If your average hours dip below the 30-hour
threshold or you lose full-time status, we reserve the right to terminate your benefits.
