Greetings!
This has reference to the concern of my client and petitioner re Petition for
Correction of Clerical or Typographical Error in the Certificate of Live Birth
under Republic Act 948 otherwise known as “ An Act Authorizing the City
or Municipal Civil Registrar or the Consul General to Correct a Clerical or
Typographical Error in an Entry and/or Change of First Name or Nickname
in the Civil Register Without Need of a Judicial Order, amending for this
purpose Articles 376 and 412 of the Civil Code of the Philippines.”

Further, the same erroneous Certificate of Live Birth shows that the first
and last name of Angelica’s mother is “XXXX” when it should have been
“XXXX” being the daughter of the late, XXX. It should be noted that XXX
has one (1) sibling and uses the last name “Paje” as well. This is confirmed
in the PSA issued Certificate of Live Birth of XXX brother named, XXXX.
Hence, a need to correct the first and last name of XXX mother from
“XXXX” to “XXXX”, to wit:
