    described motor vehicle going north bound on my way to Tarlac City, and before reaching
    the traffic light in Brgy. Anupul Bamban, Tarlac, a speeding motorcycle without sidecar
    going south bound and abruptly took a left turn without reducing his speed, as a result, he hit
    the left front bumper and fender of my said motor vehicle which cause damage;

    3. Effort to locate the persons and motorcycle who caused said damage were all in vain;

    4. I am executing this Affidavit of Vehicular Accident to attest to the truth of all the foregoing,
    and to recover from the insurance company the cost of repair and/or damage that the above-
    described motor vehicle have suffered by reason of the said unfortunate incident;
