1.0 INTRODUCTION
1.1 Objective of Contract
Pengerang Refining Company Sdn Bhd (formerly known as PRPC Refinery Cracker Sdn Bhd)
(hereinafter referred as OWNER) intends to engage CONTRACTOR to dismantling and
installing roof and wall cladding including specified accessories by OWNER at Compressor
House 1520 (hereinafter referred as WORK).
This CONTRACT is a Non-Escalating Fixed Lump Sum per module basis inclusive of all taxes,
duties, transportation, handling charges, insurance and any other related cost required for the
completion of the WORK as per specified in this CONTRACT.
The WORKS is essentially involving the supply of all experienced and competent manpower, work
supervision, tools, equipment, materials, consumables and transportation. The WORK shall be
completed within two (2) months. The WORK are mainly consisting of (but not limited) to the
following:
1.1.1 Dismantling existing roof and wall cladding, purlin gutter and rain water down pipe (RWDP).
1.1.2 Installing the new roof and wall cladding, purlin, gutter and RWDP.
1.1.3 Logistic (i.e materials, tools and equipments)
1.1.4 The CONTRACTOR’s scope excludes the following (i.e. under OWNER):
 Materials (i.e metal cladding, gutter, purlin and etc.)
 Heavy vehicle (i.e crane, boom lift and etc.)
 Scaffoldings

1.2 Area of Coverage


Area of coverage for this CONTRACT is Area 2A, (Atmospheric Residue Desulphurization Unit
(ARDS); Compressor House 1520

1.3 Definitions
1.3.1 OWNER
Pengerang Refining Company Sdn Bhd, Company No: 1058557-T (formerly known as
PRPC Refinery Cracker Sdn Bhd).
1.3.2 WORKS
Dismantling and installing roof and wall cladding including specified accessories by
OWNER at Compressor House 1520
1.3.3 CONTRACTOR
Contractor who shall be responsible to execute the WORK as defined in this document.
1.3.4 CONTRACT
The agreement signed between the OWNER and the CONTRACTOR whereby; the
OWNER agree to award the WORK to the CONTRACTOR and the CONTRACTOR
agree to undertake the WORK, as per term and conditions outlined in the CONTRACT.

2.0 SCOPE OF WORK


2.1 Main Scope of Work
2.1.1 CONTRACTOR shall perform WORKS and strictly to follow work schedule provided by
OWNER.
2.1.2 CONTRACTOR shall closely communicate with OWNER’s Civil and Structure team and
develop an integrated schedule to meet OWNER requirement.
2.1.3 CONTRACTOR shall prepare the following requirements;
a) Prepare and get approval for Job Pack documents not limited as per para 6.0, including
detailed Job Method Statement (JMS) which contain Civil and Structure works.
b) Prepare QA/QC procedure which include all the holding points as stated in JMS for
checking, method of checking and relevant specification (if any) for WORKS and
material inspection.
c) Prepare Job Hazard Analysis (JHA) together with OWNER based on agreed JMS and
QA/QC procedures.
2.1.4 CONTRACTOR to assist Pre-Cleaning Inspection of internal EQUIPMENT/TOOLS which
will be performed by OWNER.
2.1.5 CONTRACTOR shall be responsible for coordination and interface with other OWNER’s
contractor in order to facilitate the WORKs and associated WORKS for the safety of
manpower, materials, equipment and surrounding facilities i.e.:
 Scaffolding erection
 Heavy vehicles

Preparation
2.1.6 CONTRACTOR to ensure the working area are ready and safe from any of obstruction
and hazard.
2.1.7 CONTRACTOR to provide experienced workers under the direction of a qualified
supervisor who is full filled of the project requirements.
2.1.8 Prior to commencing the works, CONTRACTOR to ensure that all tools, lifting gear and
equipment to be inspected by OWNER inspection team.
2.1.9 For safety and any damages prevention purposes, CONTRACTOR to assist Scaffoldings
CONTRACTOR to erect structured scaffolding to protect equipment under it such as
pipeline, compressor and others.
2.1.10 CONTRACTOR to provide lifting plan as per work sequence installation and approved by
OWNER. Contractor to assist Heavy Vehicle CONTRACTOR in choosing the suitable
crane capacity shall be used for lifting and setting to final position.
2.1.11 CONTRACTOR shall apply PTW before start working at any place area.

Dismantling
2.1.12 CONTRACTOR to assist Heavy vehicle CONTRACTOR in dismantling of existing purlin,
cladding, gutter and rain water down pipe (RWDP)
2.1.13 CONTRACTOR shall transfer the dismantle items piece by piece to assemble area.

Site Lifting and Rigging


2.1.14 CONTRACTOR to assist Heavy vehicle CONTRACTOR to provide, but not limited to the
following:
a) Prepare lifting plan and submit to OWNER for approval.
b) A brief job method statement
c) Technical details and specification for crane, slings and shackles to be used.
d) A detailed scale plan drawing reflecting the crane position(s), equipment or
components to be lifted, adjacent structures/facilities or any other item that may result in
a hazardous condition within the potential 360deg swing radius of the crane’s boom.
e) The approval of CONTRACTOR’s suitably qualified rigging and lifting responsible
person who shall, at all time, remain in attendance for the full duration of the lift.

Installation
2.1.15 CONTRACTOR shall be responsible for protecting surrounding works, equipment,
materials and personnel from damage/injury from the execution of the WORKS.
2.1.16 CONTRACTOR shall carry out all repair works such as drilling, minor cutting, grinding,
and minor fabrication work due to CONTRACTOR negligence during execution of the
WORKS.
2.1.17 CONTRACTOR shall submit repair procedure to OWNER for approval prior to executing
any repair work.
2.1.18 Apart from the repair manpower, the scope of repair work shall also include the obligation
for the CONTRACTOR to supply all related equipment, tools, material and consumable
required for the completion of the repair work.
2.1.19 For any internal repair or replacement work, it shall be completed prior to installation
activities.
2.1.20 Repair work shall only consider accepted by OWNER once repair work has been
completed as per approve repair procedure.
2.1.21 CONTRACTOR is required to prepare daily/shift progress report and submit to OWNER
on daily/shift basis.
2.1.22 CONTRACTOR shall submit Final Report after all WORKS and housekeeping
completed.

2.1.23 CONTRACTOR shall guarantee that all WORKS to be executed to the highest quality of
workmanship, to the satisfaction of OWNER and relevant authorities.
2.1.24 CONTRACTOR shall guarantee the WORKS be completed are free from defect for six
(6) months period from the date of OWNER acceptance of Certificate of Work
Completion (CWC)

3.0 CONTRACTOR SCOPE OF SUPPLY


3.1 Manpower
3.1.1 CONTRACTOR shall provide skilled, competent and adequate number of Project
Management Team (PMT), Critical HSE manpower and other relevant manpower to
ensure satisfactory completion of the WORKS within schedules but not limited to the
following;

CONTRACT.

must be able to make on the spot decision on every
matters pertaining to the work.
 He shall be dedicated at the site at all time during the
execution period and shall be attending daily progress
meeting with OWNER supervision team. Only on
urgent / rare circumstances and with prior approval
from OWNER Manager, will he/she be allowed to go
off-site.
 He/she shall be responsible to update detail planning
and the daily progress at specific time during the
execution and to verify or take immediate action should
there be any schedule slippage as measured against
the APPROVED schedule.
 He/she shall ensure that all work proceeds smoothly
and all works Performance Measurement / Milestone is
achieved as per agreed plan.

2 Project Engineer 1  Shall have a degree in Mechanical Engineering or


(planner/scheduler/ (daily basis) equivalent qualification
coordinator)  Minimum five (5) years in supporting project
manager’s role and have experience in multi-bed
catalyst unloading/loading works, mechanical static
equipment maintenance and servicing at PETRONAS
or Oil and Gas industries.
 He/she shall be competent and responsible person
that are able to manage and control fellow workers
accordingly

3 HSE Safety 2  Minimum seven (7) years of experience as Safety


officer/supervisor (with (daily basis) Supervisor in Oil & Gas and/or Petrochemical Industry,
inert entry experience) inclusive of servicing experiences in high pressure
reactor such as multi-bed catalyst unloading/loading
works, mechanical static equipment maintenance and
servicing or similar works
 Valid certificate of DOSH registration as Safety &
Health Officer (SHO) and possess valid NIOSH AESP
inert entry.
 It will be value added and good if he/she has Certified
Professional SW Manager (CePSWAM) which is
competent person for schedule waste
 He/she required to be present at site at all time during
the WORKS to monitor the safety performance of
his/her personnel and to ensure all WORKS comply
with OWNER HSE rules and regulations and safety
procedures.
 He/she shall report direct to OWNER HSE
Representative and shall be responsible to coordinate
daily Tool Box Meeting, conduct routine/random safety
audit and attend periodic safety meeting as arranged
by OWNER HSE.
 He/she shall ensure that all safety program and
requirement outlined by OWNER is implemented and
followed by his fellow workers.
 He/she shall also ensure that equipment, tools,
vehicles, electrical appurtenances, work methods,
platforms/scaffolds, and others not limited to the above
mentioned is in good condition, installed, applied, and
used correctly

4 QA/QC 1  Minimum five (5) years experiences in


officer/supervisor (daily basis) Turnaround/Shutdown as QA/QC Officer in Oil & Gas
and/or Petrochemical Industry, inclusive of catalyst
change-out for high pressure reactor and three (3) nos
in supervisory position
 He/she shall also be at site all time during the WORKS
to monitor the overall quality standard and
workmanship. He shall report direct to OWNER QA/QC
Representative and shall be responsible to develop /
implement quality checklist plan and audit during the
execution period. He shall be responsible for the
quality monitoring system, housekeeping and control of

5 Maintenance Scheduler 1  Minimum five (5) years experiences in scheduling,

preparation of all necessary work permits, monitoring
and control of work activities and required for daily

3.1.2 CONTRACTOR shall submit propose Organization Chart with minimum number of
manpower as per table in para 3.1.1 for OWNER’s review. CONTRACTOR to
propose suitable and sufficient manpower loading to meet WORKS schedule as
required by CONTRACT.

3.1.3 CONTRACTOR shall submit a comprehensive resume of the following personnel


as part of technical proposals;

I. Project Manager.
II. Project Engineer (planner/scheduler/coordinator).
III. HSE Safety officer/supervisor.
IV. QA/QC officer/supervisor.
V. Maintenance Scheduler and Planner
VI. Work/site supervisor

3.1.4 All manpower shall have valid NIOSH OGSP pass and must attended Safety Induction
conducted by OWNER.

3.1.5 CONTRACTOR is required to follow a 24-hrs working pattern with two 12-hrs shifts

3.1.6 CONTRACTOR is required to make sure that all manpower is trained and have valid
pass (e.g. Inert Entry supervisor, Inert Entry technician, confined space (AESP), fire
watch)

3.1.7 For foreign worker/expatriate, CONTRACTOR shall apply VISA or Permit To Work from
Immigration and ensure it is ready prior the activity.

3.1.8 CONTRACTOR shall prepare additional team to support inspection and associated
activities.

3.2. Tools and Equipment

CONTRACTOR shall provide sufficient machines, equipment, vehicles, tools, and other
accessories that are suitable, adequate, and in worthy condition required for the satisfactory
completion of the WORKS. The required tools and equipment shall include all but not limited to the

3.3. Materials and Consumables


CONTRACTOR shall supply or provide adequate materials and consumables suitable for the
satisfactory completion of the WORKS. The materials shall include all but not limited to the
following:

1 Personal Protective Equipment CONTRACTOR shall provide good basic PPE to all its
(PPE) manpower such as safety glasses, safety helmet, disposal
ear plug, safety shoes, gloves and Fire Retardant Clothing
(FRC) coverall.
FRC shall be one-piece dark green NOMEX IIIA with
company’s name at back.
CONTRACTOR shall also provide disposal clothing to all its
manpower for confine space entry (e.g. Tyvex®)

2 Canvas or weatherproof plastic Use for covering the valves, nozzles, manholes, pipe ends,
instruments, components, etc.
3 Cleaning consumables Use for cleaning such as cotton rags, cleaning rags, heavy
duty garbage bags.
4 Gas badge(H2S and Hydrogen) To be used on sharing basis. Maximum usage 5 pax for 1
badge(in same specific area)
6 Fire extinguisher (with valid CONTRACTOR shall stationed at every permitted tool,
certificate from BOMBA and equipment or machine capable of producing sparks, and
pressure gauge at green position) any permitted Hot Work involving open flame and sparks,
simultaneously.

3.4. HSE Requirement


3.4.1. In general, CONTRACTOR shall strictly comply with all OWNER Health, Safety and
Environment (HSE) requirements, rules, and regulations including;
 “Zero Tolerance” HSE Guideline.
 To follow all Zeto Rules requirement during readiness and job execution

3.4.2. Non-compliance to the rules and regulation will cause the contractor’s personnel and the
CONTRACTOR to be subjected to disciplinary action.

3.4.3. All CONTRACTOR's personnel shall attend a mandatory OWNER’s Safety Induction to be
organized by OWNER HSE Department before being allowed to enter or work inside the plant
premises. CONTRACTOR shall be required to observe the briefing schedule closely. Request
for such briefing shall be forwarded via proper request form to the OWNER Catalyst Change
representative and HSE Department Three (3) working days in advance. Only personnel who
received OWNER’s Safety Pass will be provided with Gate pass to allow entrance inside the
plant.

3.4.4. CONTRACTOR’s personnel to undergo and pass medical check-up including drug screening
test. CONTRACTOR to submit medical report dully signed by approved medical officer
confirming the personnel is fit for the work.

3.4.5. CONTRACTOR shall commence any WORKS only after prior authorization from OWNER via
proper Permit to Work (PTW) system.

3.4.6. Starting 1st January 2020, CONTRACTOR must bear the cost of new card of Rapid
pass that cost RM80 per worker

3.4.7. CONTRACTOR shall ensure all of his/her worker (including driver) who will be involving in this
WORK shall possess CIDB and NIOSH Oil and Gas Safety Passport (OGSP) and shall be trained
with OWNER HSE Induction training prior to start WORK.

3.4.8. CONTRACTOR is responsible to equip his/her worker with complete personal protective
equipment (PPE) which is NOMEX IIIA or TECASAFE coverall, safety helmet, safety glass,
safety shoes and H2S gas badge.

3.4.9. Vehicle must be registered with security management for entry sticker prior job execution.
Vehicle must be equipped with a spark arrestor and fire extinguisher and inspected by
OWNER prior to entering process area.

3.4.10. Driving Safely & Traffic Regulations

a) CONTRACTOR personnel traveling to and from work site shall use proper means
of transport. Such transport shall comply with the regulatory requirements.
Transportation of personnel in open trucks is prohibited unless fitted with seats
and approved by authority to carry passengers. CONTRACTOR shall be
responsible to provide safe access and egress to the work site for green field
project. CONTRACTOR shall set allowable speed limit and place suitable road
signs at the work site.

b) CONTRACTOR shall observe that the road speed limit within COMPANY’s
premises as per speed limit signage and adhere to any road signage which notifies
on any changes in speed limit based on specific location.

c) CONTRACTOR shall ensure that in the operation of motor vehicles, the first duty
of the CONTRACTOR’s driver(s) is safety. Every precaution must be taken to
prevent accidents.

d) CONTRACTOR shall ensure that any CONTRACTOR’s vehicles during the


performance of WORKS should always be properly parked to avoid road
obstruction. Vehicles should not be parked in front of the Fire Station or Fire
Hydrants. The ignition key should not be removed and doors should not be locked.

e) CONTRACTOR shall ensure that no overloading of vehicles is permitted within


COMPANY’s premises.

f) CONTRACTOR’s personnel shall have a valid driving license issued by Road


Transport Department. All drivers of vehicles must possess a valid license for their
particular class of vehicle. Vehicles must be equipped with safety belt. Safety belt
shall be used during travelling.

3.4.11. Transportation, Vehicles and Mobile Equipment

a) Whether the travel is by land, sea or air, CONTRACTOR is obliged to follow both
OMPANY and established industry guidelines for the safe transport of personnel
and equipment. In the event that COMPANY is providing a means of transport for
either personnel or equipment, then CONTRACTOR and SUB‐CONTRACTOR’s
personnel should adhere to the instructions and requirements of COMPANY.

b) CONTRACTOR’s vehicles and mobile equipment such as cranes, lorries,

compressors, etc., must be registered, licensed, insured and maintained in good


working condition.

c) Vehicle and mobile equipment must never be refueled whilst the engine is running.

d) Private cars will not be allowed into COMPANY premises.

3.5. Communications
3.5.1. CONTRACTOR shall provide walkie-talkies to be used by their key personnel. The key
personnel shall include but not be limited to the Project Manager, foremen or field supervisors
of each sub-area, Safety Officer, and Standby men for the Confined Space Entry.

4.0 CONTRACTOR’s OBLIGATION

4.1. CONTRACTOR shall ensure that all CONTRACTOR's personnel including that of
CONTRACTOR's sub-contractor(s), once inside OWNER premises, to adhere strictly to the rules
and regulations set forth. OWNER reserves the right to penalize anyone caught violating the rules
and regulations.

4.2. The CONTRACTOR shall ensure that WORKS performed is in accordance with good engineering
standard and practices, all statutory Health, Safety, and Environmental Rules and Regulations
and OWNER 's HSE MANAGEMENT PROCEDURE.

4.3. CONTRACTOR shall send all nominated workers to attend Safety Training at OWNER premises
at least 3 weeks prior to start date. CONTRACTOR shall also have valid OGSP and CIDB. For
Work Leader, he/she must attend PTW training at OWNER premise and must pass the test.

4.4. All CONTRACTOR's personnel shall attend safety induction organized by OWNER Safety
Department.

4.5. CONTRACTOR shall work whatever hour’s necessary to meet OWNER's schedules and
requirements as determined by OWNER.
4.6. OWNER’s Engineer/Supervisor reserves the right to reject any CONTRACTOR personnel if said
performance is judged to be below expectations. CONTRACTOR to provide acceptable
replacement within agreed time frame.

4.7. CONTRACTOR shall hold a toolbox meeting (minimum five minutes) at the beginning of each
shift before starting any work. This is to ensure understanding of the scope of work, job safety
analyses, duties and responsibilities of each manpower, and also to review, understand, and
implement operational requirements for the safe execution of the work.

4.8. CONTRACTOR's site management personnel and Safety Officer shall attend the daily Progress
Meetings and any other meeting so required by OWNER during which CONTRACTOR shall
report progress of the WORKS, highlight any problem or difficulties etc. verbal and/or written.

4.9. In the event that tools or equipment provided by OWNER to CONTRACTOR become damaged
or defective, CONTRACTOR shall, on own expense, repair or replace such tools or equipment.
OWNER reserve the right to deduct equivalent amount from the Contract Lump Sum price or
withhold progress payment or both if CONTRACTOR fails to fulfil the condition described earlier.

4.10. In the event that equipment, structure, and facilities such as gratings, handrails, pavement, curbs,
foundation, concrete, etc. become damaged or defective as a result undue care or negligence
on the part of CONTRACTOR and CONTRACTOR personnel including that of CONTRACTOR's
sub-contractor(s), CONTRACTOR shall repair, replace, or make new of such equipment,
structure, or facilities. OWNER reserve the right to deduct equivalent amount from the Contract
Lump Sum price or withhold progress payment or both if CONTRACTOR fails to fulfil the condition
described earlier.

4.11. CONTRACTOR shall ensure good housekeeping of work site, rest shelters, and site cabins at all
time. OWNER reserves the right to stop all ongoing work and ask all CONTRACTOR personnel
to do housekeeping.

4.12. CONTRACTOR shall ensure that Safety tags validation and all working scaffolding shall be
routinely inspected and such inspections recorded in full compliance.

4.13. Any unused materials or surplus materials, plates, rods, bolt and nuts, and parts of EQUIPMENT
shall be returned and transported by CONTRACTOR to designated area determined by OWNER

4.14. CONTRACTOR site management personnel, area/field supervisors, and foremen shall keep
close supervision, regular reminders, give training and/or briefings, and exercise tight control over
all CONTRACTOR personnel so as to prevent unauthorized access or operation of any of refinery
equipment (i.e. valves, motors, buttons, switches etc.). Such authority rests solely with OWNER.

4.15. Consuming non halal food, alcoholic drinks, and/or narcotics, and engaging in illegal activities
such as gambling or fighting are strictly prohibited once inside OWNER premises.

4.16. CONTRACTOR shall screen and bar those CONTRACTOR personnel under intoxication from
drugs or alcohol or those carriers of any disease contagious in nature from entering OWNER
premises.

4.17. CONTRACTOR shall impose strict discipline and keep vigilant on CONTRACTOR personnel so
that they do not go astray into unauthorized areas other than those specified, sleep in the work
site, or involve in any kind of gambling or fighting inside OWNER premises. OWNER reserve the
right to forbid entry, penalize in accordance with OWNER Rules and Regulations, or even dismiss
immediately from OWNER premises.

4.18. CONTRACTOR shall familiarize and strictly follow OWNER's Permit to Work Permit (PTW)
system and procedures. A PTW must be obtained before commencement of any work and
renewed upon expiry. No work shall start without a valid PTW.

4.19. CONTRACTOR shall fully comply with Contract HSE Requirements.

4.20. CONTRACTOR shall be responsible to make sure that the CONTRACTOR’s scaffolding fully
comply to the standard specified in this CONTRACT and erect the scaffolds based on safe and
good engineering practice to the highest quality possible.

5.0 OWNER’s OBLIGATION

5.1. OWNER shall appoint dedicated field personnel for field supervision and liaison with
CONTRACTOR.

5.2. OWNER to provide Safety Training for nominated CONTRACTOR’s workers at OWNER premises
prior to the work.

5.3. OWNER shall engage relevant parties acting on behalf, or own representatives to carry out all
inspection requirements.
5.4. OWNER shall conduct post and final inspections prior to closure or boxing up.

6.0 JOB PACK REQUIREMENT


6.1. Below documents shall be submitted by CONTRACTOR to OWNER prior to start any WORKS
6.1.1. Approved Schedule.

6.1.2. Approved sticker for machinery, tools and equipment by workshop and electrical
central.

6.1.3. Approved Job Hazard Analysis (JHA).

6.1.4. Approved Repair procedure (if required)

6.1.5. Approved Manpower loading

6.1.6. Approved Job Method Statement

6.2. CONTRACTOR shall work together with OWNER in developing above documents.

7.0 WORK COMPLETION


7.1. Below documents shall be submitted by CONTRACTOR to OWNER as part/attachment of the
Certificate of Work Completion (CWC);
7.1.1. Certificate of Completion
7.1.2. QAQC Inspection Report

7.2. WORKS shall only be considered as fully and successfully completed when the work has been
accepted by approved OWNER representative.

7.3. Other requirement for the OWNER to issue Job Completion Ticket (JCT) are:
7.3.1. Submission of final service report and supporting documents as per para 7.1. All report to be
submitted to the OWNER within seven (7) working days after WORKS is completed.
7.3.2. The warranty for each WORKS shall take effect immediately after the date issuance of the
JCT and remain in force for a period of six (6) months. In the event that the delay fail to

operate within the said period due to negligence of CONTRACTOR during WORKS
execution, CONTRACTOR shall rectify the equipment at no additional cost to OWNER.
7.3.3. All housekeeping including cleaning and disposing of all rubbish, left materials, damaged
materials, etc. has been done by CONTRACTOR to OWNER’s satisfaction.
7.3.4. All damages to OWNER’s property done by CONTRACTOR have been rectified to OWNER
satisfaction.
7.3.5. All tools, equipment and materials provided by OWNER have been returned in good and
serviceable condition if any.

8.0 FINAL REPORT VDML


8.1. Below documents shall be submitted by CONTRACTOR to OWNER upon work completion which
is also known as the Final Report VDML (Vendor Document Master List):
8.1.1 Certificate of Work Completion (COC) .
8.1.2 Signed Pre Activity Safety Review (PASR) checklist/report
8.1.3 Approved Inspection
8.1.4 Signed timesheet for additional work (if any)

8.2 CONTRACTOR to produce the Hard Copy Report and softcopies (in Pdf format) within 7 working
days after completion of each WORKS complete with endorsement from each relevant parties.

8.3 All videos and photos captured during WORKS execution belongs to OWNER and shall not be
shared or published without written consent from OWNER. All materials shall be submitted for
OWNER safe keeping.

8.4 All anomalies, faults, degradation or defects noted during the work if any, shall be highlighted in
the report. Detail description of the anomalies, faults, degradation or defects and
recommendations for their respective corrective actions shall also be included in the report.

8.5 CONTRACTOR shall prepare and keep a historical record each equipment serviced, repaired and
replaced including defect descriptions, date and cost of work

9.0 BIDDER DOCUMENTS SUBMISSION

9.1. In addition to the standard document submission required by Group Procurement (GP), BIDDER
shall submit the following for evaluation purposes.
i. Un-priced and priced commercial proposal/price quotation
ii. Compliance/deviation list from OWNER terms and condition*
iii. Compliance/deviation list from OWNER technical specifications*
iv. Company HSE plan/manual with JHA examples and inert entry procedure
v. Comprehensive CV/resume of Project Manager, Project Engineer, Safety Supervisor,
HSE Safety officer/supervisor, QA/QC officer/supervisor, Site supervisor, Maintenance
Scheduler, Maintenance planner, Logistic/Material Coordinator, Work leader, Mechanical
Fitter, Riggers, Fire watcher, SMAW & TIG Welder CS/SS and helper/general worker

*If not submitted, OWNER shall assume that BIDDER/CONTRACTOR shall 100% fully comply with all the requirement
stipulated in this CONTRACT without any deviation.
