APE 2020 Guideline and Procedure

Annual Physical Exam 2020 Policy and Guideline

:All permanent employees are required to undergo Annual Physical Exam (APE) in compliance with company policy and DOLE occupational safety and
Coverage and Schedule of health standard.
Annual Examination :Annual check-up must be availed within the specified period from November 16, 2020 to December 16, 2020
:Employees are allowed to use half day working day to undergo APE provided that they inform their immediate superior.
: Basic 5 (CBC, Chest X-Ray, Routine Urinalysis, Routine Fecalysis & Complete Physical Examination (Visual Acuity, Oral Exam & Medical History))
Annual Physical Exam : ECG for 35 years old and above ( Male and Female)
(Check ups/ covered tests) : Pap smear for 35 years old and above (Female)
** APE Package may vary depending to the company eTiQa plan
:Please refer to the attached list of the accredited clinic.

:Note that dependents with eTiQa coverage can avail the same APE package, please refer to your plan if covered.
Phase 1- Making an APE appointment
1. Call the accredited and operational clinic from the list to make an appointment. Please note and follow clinic's operating hours.
2. Once confirmed, request LOG from APE eTiQa team ([email protected] ; [email protected]). Issued LOG will be valid until Dec 16,
2020.
3. Please prepare and print LOG and consent form.
Phase 2- On the schedule day of APE
4. Proceed to the preferred clinic. Present either physical eTiQa card or the digital ID in Smile app, and printed copy of LOG and signed consent
form to the clinic reception. Follow safety and health protocol of the clinic. For questions on your eTiQa number, please contact your respective assigned
HRBP.
Procedure
5. Undergo the APE.
Phase 3- Upon APE Completion
6. Ask and confirm the release of APE results.
7. Submit the APE details to HR Team to track the individual employees compliance.
8. On the submission of APE result, If APE was taken in;
: Hi-Precision clinics, an online access will be provided to you to get the results.
: Other clinics will require a personal pick up of results. You must provide HR a soft copy of the APE result summary by emailing
[email protected].
