JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, CONSTANCIA L. MIRANDA and JESSICA U. BALLESTEROS, both


of legal age, Filipinos, and residents of Bgy. Prinza, City of General Trias, Cavite, after
having been sworn in accordance with law hereby depose and state that;

1. We personally know MA. ROSELLA P. LUJERO, who is our long time neighbor
and acquaintance. She was born on January 12, 1963 and the fact of such birth
is registered before the Local Civil Registrar of General Trias, Cavite;

2. When MA. ROSELLA P. LUJERO was born, the name recorded and registered,
before the above-mentioned local civil registrar, was erroneously entered as ROSELLA
P. LUJERO instead of MA. ROSELLA P. LUJERO.

3. We know for a fact that the person by the name of ROSELLA P. LUJERO as
appearing in her Certificate of Live Birth and the name MA. ROSELLA P. LUJERO as
appearing in other pertinent records, refer to one and the same person and that her true
and correct name is MA. ROSELLA P. LUJERO;
 
3. We are not in any way related to the foregoing person within the fourth civil degree
either by consanguinity or affinity, nor do we have any pecuniary interest in this matter
whatsoever;
 
5. We are executing this affidavit to attest to the truthfulness of the foregoing statements
for whatever legal intents and purposes it may serve.

October 2018 at the City of General Trias, Cavite, Philippines.
