__Design calculations, cross sections, and method of stabilization of planned channels (include
temporary linings)
__Design calculations and construction details of energy dissipators below culverts and storm
sewer outlets (for rip rap aprons, include stone size, diameters and apron dimensions)

__Narrative describing the nature and purpose of the construction activity.
__Completed Financial Responsibility form for project. Form to be notarized and signed by
person financially responsible for project. Original copy required.
__Construction sequence related to Sedimentation and Erosion Control (latest revision). Include
installation of critical measures prior to initiation of the land disturbing activity and the removal
of measures after areas they serve have been permanently stabilized.
