Clinical Evaluation Tool

This document describes a clinical evaluation tool called a summative scale used to assess nursing student anxiety levels. [1] It uses a 5-point Likert scale ranging from "Never" to "Very Often" to rate agreement with statements about anxiety. [2] The scale contains 7 statements about experiencing anxiety in daily life, fears, avoiding social activities, panic attacks, and how anxiety impacts schoolwork and life. [3] Scores are calculated by assigning point values from 5 to 1 based on the response chosen, with higher scores indicating greater anxiety levels.
