QC Story

The QC story is a problem-solving process used by quality control circles (QCC) to systematically solve problems. It involves 7 steps: 1) identifying the problem theme, 2) grasping the current status and goals, 3) analyzing and determining the root cause, 4) planning action, 5) implementing the action plan, 6) assessing effectiveness, and 7) standardizing and planning for the future. The process provides a structured way for QCC members to analyze data, make evidence-based judgments, and develop efficient solutions to identified problems.

What is QC story ?

• The problem-solving process is commonly

referred to as the QC Story

• Each Story shows how QCC members solve a

problem in a systematic PDCA manner.

Why QC story ?

• Analysis of data

• Provide appropriate judgment according to evidence

• If existing procedure with specialized skills will give

efficient way of solving problem

Steps for QC story

• Theme Selection ( Problem Identification)

• Grasping Current status and Goal setting
• Analyze and determine root cause
• Action /Activity Plan
• Implementation of Action Plan
• Assessment of effectiveness ( Evaluation of
result)
• Standardization and future Plan
Problem Identification

Objective
To Provide focus by Systematically weighing the
identified problems

Out put
Prioritization of problem
What is a problem?

• A problem is a deviation or an imbalance

between what should be and what actually is
happening.
Type of problems

• Quality improvement
• Work simplification
• Waste reduction
• Safety
• Energy saving
• Productivity increase
Problem Definition

• Put half the efforts in looking the real problem

• Gather data to reach the real problem

• A problem well defined is a half solved

problem

• Be specific not vague

Classify the problem

• Classify the problem according to its

significance
• Significant
• Simple concern

• Problem with obvious or ready solution

• The extent of the problem is relatively small
against its target performance
Prioritization Matrix

• List of identified problem

• Brainstorming on appropriate criteria

• Put value to each criteria

• Prepare matrix

• Rank them
Sample prioritization matrix
Grasp Current situation

Objective
To Clearly specify the magnitude of the problem

Out put
Final problem statement and objective statement
Understanding the current situation

• Review the current system and standard

• Agree on the scope of the problem

• Validate seriousness of problem

• Finalize problem statement ( What, when,

where, how much)
Finalize Problem statement

• Effective problem statement

• Answer
• What

• When

• Where

• How much
Sample problem statement

• Header tube leakage in CRD condenser in 23

pieces out of 100 piece checked on WLT on 24
Sept-15 in second shift
Objective setting

• After establishing problem decide how much

improvement is possible and manageable by
the team
Objective setting

• Management direction

• Make/ Use bench mark

• Industry standard
Objective setting
Root cause analysis

Objective
To Identify root cause

Out put
Correct root cause of problem
Sample Fish bone diagram
Identify Probable cause

• Nominate probable cause of the problem

• Relation between cause and effect must be clearly
understood
• Identify the causes
• Select root cause
• Select critical /Potential root cause
• Possible ways to eliminate potential root cause
Analysis of root cause

• Identify probable cause

• Validate and identify potential root cause

• Identify controllability of potential cause

Solution formulation

Objective
To eliminate correct / potential root cause of
problem

Out put
Solution of the problem and action
implementation plan
Finding solution
• Good ideas
• Creative ideas
• Innovative ideas
• Flexible thoughts
• Critical mind
Selection of best solution
• Analysis of solution
• Solvability

• Feasibility

• Controllability

• Sustainability

• Activity Planning

• Side effect of solution

Generate alternate solution

• Give team enough time to look for possible

solution

• Allow team to source for other solution

• Take expert advice

• Identify recommended for uncontrollable root

cause
Evaluate and select best solution

• Compare advantage and disadvantage

• Measure effectiveness of solution

• Best alternative solution must reduce the root

cause
Details of root cause

• Identify the process for implementation of

solution

• Identify resource required for implementation

of solution
Prepare action Plan

• Identify the activities to be performed

• Use different chart if required

• Time frame of the project completion must

confirm to the objective
Identify side effect of the solution

• Identify the side effect of implanted solution

• Prepare preventive measure plan and

contingency plan

• Use analytical methods

Implementation solution

Objective
To perform activities as per plan

Out put
Implementation actions
Implementation of solution

• Start implementation

• Monitor progress of implementation

• Identify concern

• Initiate corrective actions

Evaluation of result

Objective
To validate effectiveness of the proposed
solution

Out put
Effectiveness of solution
Evaluation of result

• Summarize and compare the data against

target

• Identify the benefit of the project

• Summarize difficulties and learning

• Derive conclusion
Evaluation of result

• Compare before and after data

Standardization

Objective
To ensure efficient integration of the new
procedure

Out put
New standard procedure and targets
Standard activities

• Documentation –new

• Change in current documentation

• Change in formats and procedure

Benefits of the QC Story
1. Easy to understand the problem-solving processes
2. Can reflect on other activities
3. Can be disseminated horizontally for deployment
4. Facilitates accumulation of experiences and lessons
learned
5. Improves members’ analytical skills and statistical
knowledge in a systematic manner
6. Enhances individual abilities in a group orientation
