The Deans of all the constituent colleges and the Principals of all the affiliated
colleges are requested to make arrangements to send the Applications for Transcript
Card of the 2016 Batch students who are completing the UG degree programme in
2020 and for whom results have been declared, along with the check list (copy
enclosed) for further processing at this end.
All the necessary documents (Annexure) are to be enclosed with the
application of each of the students.

APPLICATION FOR UG TRANSCRIPT CARD

1. Application for Transcript Card duly signed by the student, Year Co-
ordinator and the Dean / Principal
2. Fee receipt of Rs 500 paid to the Controller of Examinations
3. Proof for student’s name - 12th Marksheet
4. Proof for Date of birth - 10th or 12th Marksheet or Birth Certificate
5. Passport size photograph with white background (2 Nos) (Hard copy)
6. Soft copy of the photograph may be sent to [email protected]
7. Proof for Parents name (Father and Mother) – Any authorized document
8. Proof for Address mentioned in the Application for Transcript Card - Any
authorized document
9. UG Provisional Certificate (photo copy)
10.Clotheslined A4 Size cover with typed address – stamp of Rs 50 for the
places within Tamil Nadu & stamp of Rs 60 for other states
11. Check list to be filled and signed by the student and verified by the Year
Co-ordinator and signed by the Dean / Principal
12.It should be ensured that the student has passed all the examinations and
has completed the credit requirements
