administration of oath on the affidavits and

d. Submit the subscribed complaint affidavit - Receiving Staff on Duty 5 minutes
and IDF with all the supporting documents Admin. Section, 8th Floor
a. Copy furnish the parties through personal - Receiving Staff on Duty 5 minutes None
2. Motion for Reconsideration and service or registered mail and attached proof Receiving Section/Admin. Office, 8th Floor
Reinvestigation of service/ mailing
b. Submit three (3) sets of document
