1. SITUATION

a. Enemy. N/A
b. Friendly Forces. HQ Army personnel.
c. Attachments and Detachments. One clinic medic.

2. MISSION. Conduct and grade the APFT on 5/7/12/14/19/21/26/28 Apr, 2011.

3. EXECUTION

Intent: A soldier’s physical fitness level is paramount to survival in a combat


environment. personnel will conduct and grade the record APFT for HQ personnel.
The APFT will be conducted to Army Standard to ensure every soldier receives fair
and impartial grading. The primary goal is 100% first time record pass for all
soldiers taking the APFT. The second is to ensure the record APFT is conducted
with the utmost efficiency, professionalism, and to Army standard.

a. Concept of Operations.

PHASE I: Army personnel arrive at Gym:

(1) HQ CMDT will ensure APFT area and run route is prepped in order to
conduct a record APFT under the direction of SSG. Set up complete at 0550.

(2) All soldiers arrive in the main foyer at the Base Gym, NLT 0600, sign in with
the APFT NCOIC and receive a DA form 705 (APFT Grading Card).

PHASE II: APFT Commences

(1) APFT NCOIC (SSG) will present APFT briefing, instructions, and
orientation.

(2) Each soldier will ensure accountability of their DA form705 until a grader
takes control of soldier.

(3) Soldiers will move to the height/weight station in an orderly fashion

(4) Soldiers needing the body fat tape test will be directed to the appropriate tape
test area with either DA form 5500-R (Male) or DA form 5501-R (Female): Male
or Female latrine located inside the base gym.

(5) At the conclusion of the weigh-in, the APFT, IAW TC 3-22.20, will
commence starting with the first event (push up) after the appropriate briefing.
The APFT events will take place in the following locations:

PT track: pushups and sit-ups.


APFT Run Route (ANNEX A): is one mile out and one mile back: run

(6) Each soldier will have a minimum of 10 minutes and a maximum of 20


minutes of rest per event.

(7) Soldiers who attempt to dispute the methods of a particular grader are to be
directed to the event supervisor, who will then direct the soldier to the APFT
NCOIC.

(8) Soldiers on profile must have approved documentation of profile status on file
with the HQ CMDT Army training NCOIC. Soldiers on profile will take alternate
APFT, if required and as indicated on the profile.

PHASE III: Post APFT Activities

(1) At the conclusion of the final event all soldiers will be released back to their
respective directorates.

(2) HQ CMDT soldiers will take all DA form 705’s to the training office for
completion and roll up.

(3) HQ CMDT soldiers will take all equipment to the training office/ISU 90 for
accountability and security.

(4) Any Soldier that fails to meet standard outlined in TC 3-22.20 or AR 600-9
will be flagged and must be counseled by the chain of command.

b. Tasks to maneuver units. None

c. Tasks to combat support units. None

d. Coordinating Instructions.

(1) Time line: Per published training calendar for Apr 2011.

(2) Use of training facilities coordinated by SSG.

(3) Uniform is Army PFU/, any necessary hot weather gear, no


exceptions. All soldiers will bring water and writing utensil.

4. SERVICE SUPPORT

a. Concept of Support.
Priority of support is to ensure that the APFT and weigh in are conducted safely and to
standards; all required equipment and resources will be made available to accomplish this
mission.

b. Material and Services.


HQ CMDT will secure the following equipment through coordination with J-4:

10 Clipboards 2 stopwatches
Extra copies DA form 705 Extra copies DA forms 5500/5501
TC 3-22.20 2 tape measures
1 Combat lifesaver bags box of pens

c. Medical evacuation and hospitalization.


Any real-time casualties will be evacuated to local medical (civilian) authorities (as
needed) after receiving initial treatment from medics. If soldiers require ambulance
evacuation 911 will be called and COS will be notified via the HQ CMDT.
