This is to apologize for the error I have made in encoding a learner’s data on the
Learner Information System (LIS). As I was enrolling Juan Dela Cruz, I got confused and
only saw his Grade 7-9 SF 10 which was from _____ (school nya dati), only to find out later
on that he transferred to Anastacio G. Yumul High School in Grade 10. His enrollment is
now pending since he was enrolled as a transferee, which should not be the case.

Attached herewith are the scanned copy of SF10 of the learner for your reference.

Again, I would like to extend my sincerest apologies for the inconvenience my mistake may
have caused. Hoping for your positive response on this matter. Thank you!
