INSTRUCTIONS TO CANDIDATES
5. A diskette/CD will be provided which must be submitted with all printouts at the end of the
examination. Note that the examination is marked from the printed output NOT from the
diskette.

SPREADSHEET
Question 1
a. Retrieve the spreadsheet DISEASE so that the first cell in the spreadsheet in A4. (1 mark)
b. Insert
(i) a column titled Total to store the total number of persons affected by EACH Disease.
(ii) a row titled total to store the number of persons affected by each year by all diseases.
(2 marks)
c. Insert the appropriate formulas to calculate the total number of persons affected:
(i) by each disease for the four year period; and (ii) each year by all disease (2 marks)
Use the range A15:A17 to specify what the values in Range B15:B17. (3 marks)
e. Using larger font size than that is used in the rest of the spreadsheet, enter the title “Waterland” in
Cell A1, and “Health Report: 2000 – 2003” in cell A2. (3 marks)
The two title rows must be centered across the columns used by the spreadsheet. Bold these rows.

f. Insert the date in the cell F3 using the date function. The date must be entered in the format
dd-mm-yy. (2 marks)
g. Insert the function that would check the total by disease and enter the text “Normal” if the number of
people with the disease is <5000 else the text “Abnormal”. Use a new column titled “Status” to store
the results of the test. 2 marks)
h. Format the spreadsheet using the comma (,) format with decimal position for ALL numeric columns.

i. Sort the data in the spreadsheet, so that the disease are arranged in descending order by the total
number of persons affected by each disease for the four year. (1 mark)
j. Set up a criteria range and an extraction rang in a blank area of the spreadsheet to find the disease
with “Normal” status. Extract the matching records to the extraction range. (3 marks)

k. Insert a border around the range A15:B17. Resave the spreadsheet as DISEASE1. (1 mark)
l. Create a Column char to show the yearly number of persons affected with disease. The chart should
be titled “HEALTH STATISTICS: 2000 – 2003”. Each column of the Chart should be labeled to
indicate the year it represents, or a legend should be provided. Axes must be appropriately labeled.
Save as YEARLY. (4 marks)
m. Create a Pie chart to show the total number of persons affected by each disease for the four-year
period. The chart should be labeled “DISEASE”. Each slice of the pie should be properly labeled to
indicate which disease it represents or a legend should be provided. Each slice of the pie should
indicate the percentage of total number of persons affected by all diseases for the four-year period.
Save as “DISCHART”. (4 marks)

DATABASE MANAGEMENT
Question 2
In order to submit the findings to the senior health personnel, the consultant compiled the list of the
health personnel and stored them in a spreadsheet called PERSONNEL.
a. Retrieve the spreadsheet file titled PERSONNEL as a database table with the same name. (2 marks)
b. Insert a field “Region” using the appropriate field type and size. (2 marks)
c. Insert the values 1 to 7 respectively for the Region field. Resave as PERSONNEL. (1 mark)
d. Create another database table called RECOMMENDATIONS. The table should contain the
following data:
f. Generate a report to show a list of ALL senior health personnel. The report should contain the Title,
FirstName, LastName, Designation and Region Name fields. The data in the port must be sorted in
alphabetical order by last name. Appropriate wording of the field names should be used to clarify the
meaning of the data items being printed. The report should contain the title:

b. Insert footer on EVERY PAGE with the text “Keeping your nation in Top Health”. The text should
be in italics and centered. (2 marks)
c. The company name should be centered and bolded with 20-Point Aril Font. (2 marks)
e. The text in the body of the letter MUST have 1.5” line spacing. (1 mark)
f. Prepare the letter, Health Services Inc. on the next page, in preparation of mail merge, using the
database table Personnel as the data source and save as MAIN. Following the instructions given
above in formatting the document MAIN. (12 marks)
h. Insert the Pie chart DISCHART in the position indicated in MAIN. (3 marks)
i. Insert the database report HEAOFF in the position indicated in MAIN. (3 marks)
j. Using the database table PERSONNEL, as the data source, and merge MAIN with PERSONNEL.
Save the results as MERGELET. (2 marks)
