NARRATIVE REPORT ON
GUIDELINES ON THE PREPARATION AND CHECKING OF
SCHOOL FORMS

School Forms summarizes pupils’ performance. Grades or numbers, like all


symbols, are an efficient way to do this. Because the parent cannot be expected to
review all the pupils’ work and arrive at all appropriate meanings, the professional's job
is to make meaning from the work and present facts, judgments, and prescriptions in a
user-friendly form. The key to school forms change, then, is to ensure that grades,
scores, or any other system can be effectively translated by parents. Comments may
well be desirable. They provide rich, insightful detail, but they do not replace the facts
about performance that are summarized in scores and grades.

In accordance with the Department of Education Order No. 11, s. 2018, Pandi
Residences Elementary School conducted a SLAC Session about accomplishing and
guidelines on the preparation and checking of school forms. The Department of
Education (DepEd) hereby prescribes the standard process and protocols in the
preparation, evaluation, and updating of school forms conducted at the end of every
school year to provide a reliable assurance mechanism of learner information, ensure
the quality and timeliness of school reports, and reduce the resources spent for clerical
and records management. This Policy adheres to the Department’s vision, mission, and
core values, and subscribes to the principles of accuracy and reliability of data,
efficiency, and accountability.

This Policy shall guide personnel involved in the preparation, checking, and
updating of school forms through the LIS in DepEd offices across governance levels,
public and private schools, and state and local universities and colleges (SUCs/LUCs)
offering basic education. This Policy also provides an assurance mechanism to ensure
that data and information being managed and generated through the LIS are updated
and validated at both the School and Division levels.
The SLAC Session was conducted to keep abreast the school with the latest
trends and best practices in accomplishing school forms and records that has a great
impact in teaching and learning in today’s dynamic education industry. The SLAC
Session has not only provided the participants and opportunity to a new skills and
knowledge, but also encourage them to make positive changes around them. The
participants also got an opportunity to share their knowledge and experience with
colleagues and learn new pedagogical approaches.

The SLAC ended with the commitment of each teacher to pursue better
education for the learners of Pandi Elementary School. Indeed the school checking of
forms may have been fruitful and productive.
