The Modified Checklist for Autism in Toddlers (M-CHAT) is designed to screen for early identification of

autism spectrum disorder (ASD) in toddlers over the age of 12 months. Ideally, it is given at the 18-24 month
well baby check. Parents complete the items on the checklist independently or by interview. Meeting the
criteria listed below, suggests the risk of ASD and indicates the need for referral to the local school district
through Early Development Network Services Coordination.

M-CHAT Scoring Key


Instructions: Compare the completed form with this key. Each response that appears in bold on this form is
counted as a failed response. Critical responses appear in italics and bold. These items are: 2, 7, 9. 13, 14,
and 15. A child fails the checklist when 2 or more critical items are failed OR when any three items are failed.
