SOP FOR GOOD DOCUMENTATION PRACTICES

PURPOSE- To lay down the procedure for good documentation practices.

SCOPE - This sop is applicable for the all quality system documents. Also this sop
provides procedure for correcting errors in documents if any.

RESPONSIBILITY – All employees those makes entry in quality system


documents.

PROCEDURE –

1. All entries made with blue ink in clear and legible handwriting.
2. Do not leave any column empty in record/document. If any column is not
applicable then write ”NA”. If no comments then write NIL or put dash.
3. Date should be expressed as DD/MM/YY OR DD-MM-YY OR DD.MM.YY .
4. If any page left blank then draw line across the page from left top to right
bottom of page with remark “Cancelled” OR “NA” with date.
5. Pencil entries not allowed.
6. Do not use any correction fluid in documents.
7. All personnel should avoid errors during data entry.
8. If any case errors should occurs then same should be corrected as described
follows-
 Do not over write the wrong entries simply cut out the wrong entry
with line permitting the reading of entry.
 Clearly write the correct entry near the cross out & put sign.
 If entire paragraph/page/line to be deleted from sequential record
then cross out the line and write a comment explaining reason for
deletion.
SOP FOR GOOD LABOARATRY PRACTICES

PURPOSE – To lay down the procedure for guideline of microbiology laboratory.

SCOPE – This procedure is applicable for microbiology department

RESPONSIBILITY – Microbiologist.

PROCEDURE –

1. Personnel must not wear cosmetic ,nail polish or any jewelry.


2. Before entering the lab one should wear lab apron, cap and masks.
3. Masks and gloves necessary during work under LAF.
4. Entry in microbiology lab should be restricted and only authorized persons can
enter into the lab.
5. Persons with physical illness should not be entered in lab.
6. All the doors of respective areas should keep closed.
7. Cleaning of lab should be done before work starts and at the end of the day.
8. Before start and after completion of microbial work wash hands up to
forearms with disinfectant solution.
