    2. Individual Performance Commitment for the period January One month after the approval
    to June 2020 & July to December 2020 (Central Office-HRMD of OPCR
    & ROPOTI-FASD)
    3. Updated Personal Data Sheet (Revised Form 2017) with February 28, 2020
    photocopies of latest training certificates (Central Office-
    HRMD & ROPOTI-FASD)
    4. Statement of Assets, Liabilities and Networth as of February 28, 2020
    December 31, 2019 (Central Office-HRMD & ROPOTI-FASD)

    The submission of HRMD & FASD of the SALN as reviewed by their respective SALN
    committee is as follows:
