• 22% of the world population suffer from hay fever
• 235 million asthma patients worldwide
• in Germany ~30% of the population suffers from
allergic diseases (2013)
– 23% of children (0-17 years) have atopic allergies (KIGGS
study, 2014)
• 13% neurodermatitis
• 11% hay fever
• 5% bronchial asthma
• Likely a high number of unreported cases
• 10% allergic contact dermatitis
• 20% food allergies
• 41% allergic sensitization to at least one antigene

Allergy - Definition
Allergy
allos = different and ergon = effect, reaction

• The body reacts differently or hypersensitive to a


specific substance (allergen), which is usually not
harmful
• Inflammatory processes (often chronic) at the
boundaries of the human organism:
 skin
 respiratory tract
 intestines (bowels)

 pollen from shrubs, trees and grasses
 animal hair
 dust mites
 food allergens like milk, eggs, different types of
grains, nuts, fruits, …
 insect poison
 medicinal products, preservatives, pesticides

Types of Allergens
Classification of allergens in four groups:
• Inhalation allergens
 e.g. pollen, fungal spores
• Ingestion allergens
 food allergens, e.g. dairy products, strawberries
• Contact allergens
 e.g. nickel, latex
• Injection allergens
 animal poisons like bee venom, snake venom
 procaine, X-ray contrast medium

Allergic Reactions –
Allergic Reaction Type 1 (immediate type)

• First allergen contact


 initial sensitisation phase
 formation of IgE antibodies
 binding of circulating IgE antibodies
to mast cells
• Renewed allergen contact
 antigen-antibody reaction
 release of histamine

Allergic Reactions –
Allergic Reaction Type I (immediate type)

• Within seconds to minutes allergic symptoms occur


 itching
 sneezing, nasal discharge
 edemas
 hypotension and constriction of the airways, at
worst leading to
 anaphylactic shock

Allergic Reaction Type II (cytotoxic
hypersensitivity reaction)
• IgM- or IgG-antibodies bind to cell-mediated antigens
 activation of
 cytotoxic killer cells
 complement system
 after hours or a few days the antigen carrying cells
are dissolved
• Examples for a cytotoxic hypersensitivity reaction are:
 some haemolytic anaemia
 agranulocytosis
 (transplant rejection)

Allergic Reaction Type III
(immune complex type)
• Deposition of circulating immune complexes in/on
joints and/or different tissues, often close to blood
vessels
 unknown cause
 activation of complement system with subsequent
tissue damage
• Examples:
 allergic alveolitis
 medication allergy
 systemic lupus erythematosus
 immuno-complex glomerulonephritis
 e.g. after streptococcal infection

Allergic Reaction Type IV
(delayed reaction type)
• Sensitization of T-lymphocytes after first allergen
contact
• After renewed allergen contact, different cytokines
are released by T-lymphocytes
 activation of macrophages with subsequent tissue
damage
• Delayed reaction
 1-3 days after allergen contact
• Examples:
 contact allergy to nickel, latex, plants etc.
 transplant rejection

Pseudoallergy
• Reaction to allergen– without sensitization
 the allergen seems to act directly on the mast cells
• No clinical distinction to a „normal“ allergy
 like an allergic reaction type I (immediate type)
• common trigger
 medicines
 food additives

Cross-Sensitivity

• Conformity with the molecular proteine structure of


an antigen causes an allergic reaction even without
previous contact
• e.g. pollen associated food allergy
 Sufferers from birch pollen allergy often can‘t
tolerate:
 nuts
 fruits of rosaceous plants like apples, pears
 mugwort is a chief allergen for herb and spice
allergies

• Development of the gut microbiome is a multifactorial
process:
 diversity of bacteria evolves until the age of 3 - 4

• Intact microbiome
 development of the immune and nervous system

 significant influence on physiological, metabolic and
psychological processes in the body

• An active immune response against commensals

Patients with a dysbioses in the intestines often suffer

Allergic Rhinitis
• A chronic, lgE-induced inflammatory disease of the nasal
mucous membranes

Atopic Dermatitis - Neurodermatitis
• Chronic inflammatory skin disease, partially
accompanied by intense itching
• Often begins in infancy
 10 to 15% of preschoolers are affected
• Elevated IgE-levels in serum, triggered by inhalation,
contact and ingestion allergens
• Symptoms
 generally dry skin, with reddened inflamed areas (eczema),
often very itchy
 complications through skin infections
 laminar swelling and coarsening of the skin (lichenification)
 nodules and pustules
