I hereby acknowledge the receipt and approval of the project entitled E-Hero to be

conducted from February 22 until May 16, 2020 at Barangay Bolbok, Batangas City. I have read

and acknowledge that the above-mentioned community project is in line with the requirement of

the National Service Training Program (NSTP) of Batangas State University. I acknowledge that

the Local Government Unit of Barangay Bolbok, Batangas City will commit its counterpart in

providing a safe and sound location and contribute whatever available resources needed for the

success of the program.

The issuance of this document is upon the request of the project proponents to whatever

legal purpose it may serve.
