AFFIDAVIT OF DISCREPANCY
I, [name], of legal age, [civil status], Filipino and a resident of [address], after having been duly sworn to in accordance with law, do hereby depose and state that:
1. I was born [name] on [date] in [hometown address] to parents [names of parents], who were already married at the time of my birth. Attached as Annex “A” is a copy of my Philippine Statistics Authority (PSA) Certificate of Live Birth.
2. My last name appearing in my Marriage Contract is misspelled as [misspelled name]
instead of [correct name]. Also, the first name of my mother appearing in the same Contract is misspelled as [misspelled name] instead of [correct name]. Attached as Annex “_____” is a copy of my Marriage Contract.
3. The said erroneous entry in my Marriage Contract relate to the correct information found in my Certificate of Live Birth. The dissimilar entries refer to the same persons.
4. I am executing this Affidavit to attest to the truth of the foregoing.
