1 Did You Know? Long Term Care is Not Cheap!
FACT
According to the Genworth 201ϴ Cost of Care Survey, a private
room in a nursing home in the Central Valley costs $8,ϴϴϮ per
month. In Los Angeles, it is $9,Ϯϳϳ per month and in Sacramento,
$10,ϭϱϵ per month. How long can you pay that?

The average monthly costs for assisted living across California is
$3,ϰϬϱ monthly.

At an average of $25 per hour, home care can cost $600 a day or
$4,200 a week!
38% of people wrongly think that Medicare and their health insurance will pay for long-
term care services.
Medicare pays for rehabilitation (up to 100 days) after a senior has been in a hospital for at
least 3 days and then goes to a nursing home for rehabilitation.
The first 20 days of rehabilitation care are completely paid for by Medicare.
Days 21-100, Medicare will pay a majority of the bill--- but not all. On day 21, the patient is
required to pay $16ϳ.50 daily either out of pocket, by their HMO insurance, or Medicare
supplement. If the patient stays the full 100 & pays out of pocket the daily rate of $16ϳ.50, the
patient will have paid $1ϯ,ϮϯϮ͘ϱϬ to the facility.
Medicare requires that you are either getting better or worse to receive rehabilitation
benefits. Once you are deemed “STABLE” --- you are responsible for all costs.
Even though the maximum period Medicare will pay for rehabilitation is 100 days, most families
will find themselves receiving a notice of termination of Medicare benefits around week three or
the 21st day. The nursing home’s business office will tell you when Medicare will stop paying.
Medi-Cal has several different programs. There is a Medi-Cal health insurance program
and also a Medi-Cal program for Long Term Care. The most misunderstood is the Medi-Cal “Long
Term Care” program.
Most people first hear about Medi-Cal Long Term Care coverage when a loved one is
hospitalized, then discharged to a rehabilitation or skilled nursing facility. Medicare may pay for
this extended care for a while…but not long.
For those who do not have long term care insurance, the choice then becomes to pay the
cost themselves, over $7,000 per month, or apply for Long Term Care Medi-Cal.
Few could afford to private pay for long without seriously impacting the financial security of
the spouse and family. Both married and unmarried individuals also worry as the estate they
intended to pass to their children or heirs is rapidly eaten up by nursing home bills.
A lot of misinformation is out there about Long Term Care Medi-Cal. Some of this
information comes from Medi-Cal workers and friends and family that mean well but are
not familiar with ALL the rules of Long-Term Care Medi-Cal.
“Most couples already qualify for Long-Term Medi-Cal”
Many people believe that they must spend down their assets in order to qualify.
Married couples can have a home AND $12ϯ,ϲ00 in assets and still qualify for Long-Term Care
Medi-Cal. Even more, if you have an IRA that is making a distribution--- the principal will not
be counted as an asset for eligibility purposes. Before incorrectly assuming you will not
qualify for Medi-Cal, please consult with QLS.
Qualifying for Medi-Cal can be confusing and quite frankly overwhelming. We do not recommend trying
to become eligible yourself since an improper transfer of assets can cause a period of ineligibility of up

Assisted Living Waiver Program
The Assisted Living Waiver program gives Medi-Cal eligible adults the
opportunity to live in an approved assisted living facility. It is an alternative for
people who need long term care, but not a nursing home. Assisted living offers
residents more independence, privacy, opportunities for social interaction, and
services more tailored to their needs.
Eligibility
In order to qualify individuals must meet three requirements:
First, the applicant must need help in order for them to live safely at home;
Second, the applicant must be blind, or disabled, or be at least 65 years of age; and,
Third, the applicant must qualify under specific financial eligibility guidelines. All individuals
who are recipients of Supplemental Security Income (SSI) automatically qualify.
1. The veteran requires the aid of another person in order to perform personal functions
required in everyday living, such as bathing, feeding, dressing, attending to the wants of
nature, adjusting prosthetic devices, or protecting himself/herself from the hazards of
his/her daily environment
2. The veteran is bedridden, in that his/her disability or disabilities requires that he/ she
remain in bed apart from any prescribed course of convalescence or treatment
3. The veteran is a patient in a nursing home due to mental or physical incapacity
4. The veteran is blind
• The deceased veteran served at least 90 days of active military service 1 day of which was
during a war time period (see Page 7 for specific war time dates). If he or she entered active
duty after September 7, 1980, generally he or she must have served at least 24 months or the
full period for which called or ordered to active duty. (There are exceptions to this rule.) ,
AND
• You are the surviving spouse or unmarried child of the deceased veteran
This benefit is only available to veterans or their surviving spouses who meet the following criteria:
An Honorable Discharge from a branch of the U.S.A. Armed Forces (Including Guard)
At least 90 days of active duty military service.
At least 1 day of the 90 must be during one of the following periods:
• Service in the Merchant Marine during WW II counts the same as naval service providing
there was 90 days at sea.
• Women in the WWII military, including nurses, qualify as veterans.
• Reserves and National Guard are not qualified unless they served 90 days active Federal duty
service with 1 day during a period of conflict. Training does not count as active duty.
• The 90 day rule is waived for those who served less than 90 days but were awarded the Purple
Hearts, suffered a service connected disability, or were killed in the line of duty.
These are just a few of the many matters you can control with the right estate planning
documents. Without an estate plan, these and other important decisions will be made by courts,
legal statutes, or individuals who may not be the people you would have selected. These may be
some of the most important decisions in your life. They can affect you significantly if you
become disabled, and affect your loved ones after your death.
How can you avoid this? Avoid the court system by getting proper documents in place.

Will
A will allows you to direct who will inherit your property upon your death, and can simplify the
probate process. Furthermore in a will you can *nominate guardians of your minor children.
A will doesn't cover everything. Your will only affects those assets that are titled in your name at
time of death for which there is no designated beneficiary.
If you die without a will, the California laws will determine how your assets are distributed. This
may or may not match your intentions regarding the transfer of assets.
Trust
A trust is an arrangement under which one person, called a trustee, holds legal title to property for
another person, called a beneficiary. You can be the trustee of your own living trust, keeping full
control over all property held in trust. The main advantage of making a living trust is to spare your
family the expense and delay of probate court proceedings after your death.
Some other good reasons to establish a trust beyond probate avoidance:
Government programs are not always the answer when it comes to paying for Long Term Care. In some
cases the reimbursement isn’t high enough or it simply will not cover the care that is needed.
People are living longer and the cost of Long Term Care is rising, there is a better than average chance that
you or a loved one will need some level of assistance.
Self-insuring gives you the flexibility and freedom when it comes to deciding what type and where you
receive the care.
Insurance will allow you stay at your home, or possibly an assisted living facility, which in many cases will
not be covered by government programs.
