    KNOW ALL MEN BY THESE PRESENTS:

    We, HERMENIA GARGAR ALICAYA, legal age, Filipino, married, a resident of P-2, Dona Helen, Province of Dinagat
    Islands, after being duly sworn to in accordance with the la, hereby depose and state that:

    1. That I am the authorized representative of my son HERMELITO GARGAR ALICAYA, with cellphone
    number 09093871493 (hereafter referred as ‘principal’) to claim, receive of the UNCONDITIONAL CASH TRANSFER (UCT)
    benefits from the Department of Social Welfare and Development (DSWD).

    2. I acknowledge that a special power of attorney to be issued by the principal in my favor is necessary to
    give my appointment any legal effect

    3. I am executing this affidavit of authority reprehensive for the purpose of obtaining provisional
    authority to transact UNCONDITIONAL CASH TRANSFER (UCT) for and in behalf of the principal until such time as a
    special power of attorney is issued in my name

    HEREBY GRANTING and GIVING unto our said attorney-in-fact full authority to do all that is necessary in or about
    the premises, as fully to all intents and purposes as we might or could lawfully do if personally present and hereby
    ratifying and confirming all that our said attorney in fact or her substitute shall do or cause to do done under and by
    virtue of these presents.

    IN WITNESS WHEREOF, we have hereunto affixed our hands this 25th day of February 2020, at San Jose, Province of
    Dinagat Islands, Philippines.
