Education Science and Technology Inc., and I do so at my own risk. In consideration for enrolling, I
understand to the following:

1. I may be subject to the risk of my personal safety, death, and/or damage of my property, and I
accept these risks.
2. I agree that the school's employees or trainer are not liable for any injury and/or damages I
sustain while in the school's vehicle or property.
3. I agree that it’s my liability if I cause any damage to the school vehicle.

I have read and understand the contents of this document and I sign it of my own free will. I also can
revoke this agreement at any time before my driving lesson and training begins, but once the training
begins, this release cannot be rescinded.
