1. I am an applicant for Senior High School Teacher position.


2. I am executing this omnibus certification to ensure authenticity and
veracity of all documents submitted (transcript of records, certificates of
outstanding accomplishments).
3. I assure full responsibility and accountability on the validity and
authenticity of documents submitted and I understand that any violations
will automatically disqualify me from the selection process.
