Database Programme Surgery


Introduction: I have now understood the database going to design 3 tables with PKs &
FKs into 1-2 Many relational whereas another appointment table will have a Non-relational.
Before I make installations I will need H/W, S/W, resources to create my database however
need a LAN server to contain 50 records in my database through the centralized location.
Statement: I am bothering to make the system because my manager has told me
to plan reorganization by the health services to increase the number of patients and
doctors manage volumes of patient data that need to upgrade the system so we
won’t get any faults or issues in the database.
User Requirement: My database will be shared by people to access the records
therefore I will need good navigation to have hyperlinks or buttons however need the data
input control which is a security feature to secure the data for e.g. Input Mask, Validation
Rule gives us an error message, Y/N, field size, how many characteristics from 255 and drop
down list is a limited number of options to choose but we need to back up the database in
case we lose it to do regular in 4 hours.
 Receptionist: A person who greets a patient will look up their appointments they have
attended to and answer telephone calls if not attend from the doctor therefore can’t access
medication information where is unauthorized for a receptionist but they can amend
personal records by changing their details if they have a brand new number or address if the
patient has gone to new location.

Doctors: A doctor is confirming who you are by checking in the database where will ask
the patient of their symptoms then talk about it typing in the database there will diagnose
you making a checkup of your previous history giving prescribed medication with advice
notes in the database will print out a prescription for the patient.

Nurse: The nurse supports for doctor to help the patient's general health by checking their
blood pressure, Cholesterol and vaccinations in a way to keep the condition stabilize but will
check the previous medical of he or she having it by confirming in the database therefore
have different types of roles under jurisdictions independently by variety of settings.

A description of the relational database: I will be showing you how to design a


relational database with PKs & FKs in three tables linked together. The first is a patient table
which has a PK then second is a Treatment table with a FK and finally third is Doctor/Nurse
table PK but all tables will have the same field name.

Designing the Appointment system: I am going to design appointment system


because the patient will have to attend their treatment where booked it on the exact
date/time from the doctor but this is a single table which called Non-relational
database has no PKs and FKs that I am not linking the three tables with appointment.

1. Patient will have 1-2 Many treatment when attending it where is necessary to have a
treatment from the doctor given them notes about their condition will get a
prescription of the medication.

2. Nurse can have many doctors to supporting them from he/she, but the doctor
will assign the nurse to help out in any situation where they get instructions by
the doctor.

Validation and verification procedures: The validation rule is an error


message given by the security feature which is control mechanism data input that

for a manual or electronic system that the doctor/nurse and receptionist will check
in the database to ask questions from patient and reply back to them.
Two data-entry forms: To make a form you will need all the field names in the
patients table to create a complete record but without the headings it will give us
incomplete record. The form is the 2nd element of a database in data entry for e.g. have
patient details taken from the table is a standard default layout for a form.  
Create a Main Menu: I am going to create a main menu diagram with all the
buttons which has hyperlinks that are Table, Forms, reports and queries below:

Five Queries: Queries are extracting data from the table which makes calculations giving
us a result but I will have to make 5 queries from my table or report.
1. How many patients have gotten a repeat prescription?
There are 7 patients who have the repeat prescription from the query in a database.
2. How many males in the patient details?
There are 13 males in the patients details.
3. How many patients attend for their appointment?
There are 6 attended for the appointment with the treatment having.
4. How many conditions are there?
There are 10 conditions that patient has from the treatment table.
5. How many females are in the patient details.
There are 12 females from patient details in a query
Three Reports: Reports is the 4th element of the database which takes data their can
make decisions to analyse it or summarize the information that is pulled from the tables or
queries where can design the report such as labels, headings and graphics
1. My first report was the treatment table where I took the data that they have to
analyse the patient's condition where I had to give them medication prescription.
2. The second report was able to find out if a patient has booked their appointment by
the receptionist there to be registered for seeing a doctor with date and time.
3.  For my final report I had to extract the data from a query that the patient has a
condition of their symptom.
