General
1. Emphasis on site changes around site and day to day operations due to COVID-19 must be
continuously carry on until it is adopted uniformly. Risk assessments should be thorough and
take into consideration the necessary precautions and risk. Foremen to ensure risk
assessments are done properly. Social distancing should be followed while carrying out work.
2. New maintenance plan has been sent out that outlines the 3-day configuration on
Wednesday, Thursday, and Friday for 6 hrs stoppages on each day. Please read carefully and
discuss with artisans, lock out procedure will have to be followed thoroughly as there will be
running equipment and standing equipment. This was done to limit amount of people onsite
and so that employees do not share accommodation while onsite. Weekly maintenance will
have to be flexible according to the work that must be done and resources available. EPCMM
vendor is being created, they need to be trialled and if happy with their standards then we
can use them, please follow up with extra people for this week. Site services and CMS to help
with plant work.
3. Planning must be held on Sun at 10AM and Tuesday 10AM, planning format amended to
include COVID-19 rules, supervisor, and non-permanent contractors and who will be
responsible for them. CMS, Consulmet, Metallurgist, Process Sup, Planner, Eng Foremen,
Safety Officer, Recovery Technician, Eng Manager or GES, Instrumentation FM/Tech,
Electrician must attend the planning meeting. Please make sure all jobs are over inspected
especially for lock out! Meeting must be conducted in English.
4. COVID-19 Procedures will be emailed to supervisors and foremen and should be discussed
with the teams for compliance.
5. Fogging as outlined in the sanitization procedure, dedicate one artisan a day to Fog all areas
in plant, maybe good job for nightshift artisan.
6. Foremen to close out safety deviations from daily safety officer report (All) – All contractors
are the responsibility of the person hosting them. The reports was emailed to everyone as
there are many closeouts needed especially for Consulmet.
7. Foremen must personally check safety files and make sure it has been approved prior to the
job.
8. Foreman must not sign any hot work, confined space, work at heights, excavation permits,
without first checking that the risk assessments are properly done, and lockout procedure is
included and correctly followed.
9. Costs must be managed as we are in difficult markets and we need to keep costs down.
10. Foreman must not sign service reports before receiving job cards from contractors.
11. Emphasize on housekeeping, spillage plan has been drawn up and must be followed!
12. Our Utilisation targets for 2020 financial year is 85% and 83% on long shut months, we have
achieved 83% overall in 2019 so much focus is needed to consistently achieve this.
13. South African Contractors should all go through screening and PCR Tests and produce up to
date results before being granted go ahead to travel to site and access into the mine.
14. Preparations for the signage with the new names has started. The two big boards that show
the plant layout have been put up in the plant near the boardroom and at the entrance of the
boardroom. The incoming shift should familiarize with the new names and start with
preparations, actual installations of the signs will gradually start on the following shift, doing
the changes in sections and not in one go. This was started and stopped due to resources
been isolated. Attendance registers must be signed by all process and engineering for
explanation of new names as part of change management.
15. EMC Trainer will help train/refresh our Telehandler drivers (6 months) and PTOs will be
signed off by Engineering Manager until arrangements can be made for the Usual trainer to
cross the border. Only a few complete as the rest were in isolation.
16. Updated handover feedback must be individually sent to the planners latest on Saturday.
17. Rootcauses are the worst that I have seen in a long time, please give this attention, I don’t
want the immediate cause, I want the root cause.
18. Everyone must understand that they are paid to work 21 days a month and that they need to
assist everywhere possible as they are only work 16/17 days per month.

created- To start printing from Thursday

5. Drawings for new septic tank to be obtained and materials ordered, quote has been
processed for structural engineer’s drawings – Drawings received and proposed to engage
contractor base on scope on work. Contractor (Mampodi) to submit quotation on the 10 th
Jan. Three quotes received, a request for purchase order to be raised.
6. Please ensure good site housekeeping, start getting rid of unnecessary containers and old
spares/scrap lying around. Plumber’s workshop roof to be removed.
7. Water operator interviews to be completed and person appointed – readvertise
8. Jobcards must be loaded on Onkey – Site services – Sewerage Treatment plant and Artisan
daily tasks submitted, to be loaded. More MSI’s acquired, Jobcard creation in progress. –
complete, to start issuing
9. XRT bunker water ingress issue to be resolved. POHLS20000185- Rubber sealant has been
ordered.
10. Water Treatment Plant PLC. Follow up on delivery date – Was sent home because no safety
file, please follow up on safety file. Awaiting Safety file to be submitted
11. Repair holes on plastic lining on process dam. Vendor created. Awaiting safety file
12. Spares ordered for repairs to pipes, follow up on delivery
13. Area under pumps behind blue tank at plant to be cemented

1. Arrange shortlist for MV electrician. To be finalised this week. On hold.
2. CAT 1and Perkins are down. Plan for base box programming on CAT 1 and repairs on the
diesel heaters on the Perkins. - Done
3. 8-way RMU delivered, please plan installation
4. West Valley rental generator to be sent back - Done
5. HHK audit completed, please get final report and start actioning – In progress, no PVC
conduit- Awaiting Delivery of conduit
6. Recovery reticulation is not clear, and drawing must be completed, Recovery to be
prioritised. In Progress.
7. Jobcards must be loaded on Onkey.
8. Cable detector training to be arranged.
9. Transformer bays, bund walls and roofs to be constructed. MCC 3&4 transformers needs
urgent attention. In progress. Please get quotes for the roofs over the transformers. MCC
5&7 done. Roofing quotes received and sent to Jan Bronkhost
10. MCC 2 220V transformer to be installed. Please trace control circuit properly before

11. LEC meeting held on the 17th Jan 2020- Awaiting minutes and confirmed date and venue for

2. Make sure Downtime dashboard downtime are correctly allocated and should be same as
maintenance KPI’s, updated and is been checked from time to time…… ongoing.
3. Planners must make sure root causes are filled in and filed, check all root causes for the
month, RCA document not filled culturally, both Process and Engineering Foreman’s to assist
drive the process, ongoing.
4. Earth leakage Jobcards to be updated according to I-Control reports
5. Please get other reports functional in Onkey asap especially artisan utilisation. New module
loaded which may give all reports needed, Marcel to go for training from the 24 th Feb where
he will learn how to use this new module. Mock-up reports to be send to Onkey as a sample
of required report structure as well.
6. Send incorrect gearboxes back to Bonfiglioli and get credit – 4 gearboxes left to order, spoke
to Quintin to swop 5 that is not needed – Done. Are they converting them free of cost?
Where is the quote for 3 gearboxes sent?
7. Marcel to finish up bus business case - 85%, please get updated quotes. Follow up Ashwin for
bus quotes.
8. All shift Jobcards to be returned/received before going off shift – 100% Jobcard attainment
compulsory
9. Post shut meeting should be held on Thursdays and minutes taken and distributed to all
stakeholders including the off shift.

1. Arrange meeting for Sandvik to discuss way forward with Ter C as gear and pinion was
changed but noise and heat is still there
2. Familiarise employees for new conveyor names – Get attendance registers from process and
engineering
3. Arrange weights for tertiary MVT screen – MVT is already machining weights for the screen,
they will confirm completion and deliver to site. - Installed
4. DVM must be done weekly and monthly
5. Procurement meeting to be held on Thursdays
6. Fix dashboard graphs – still not satisfactory – Foremen need to improve asap.
7. Decision on backup plant air compressor to be made – repair old or buy new (upgraded size)

Installation awaiting two-day shut. Install on Wednesday with new belt.

11. Follow up of LT125 pecker repairs – awaiting a cable, the delivery to site expected this week,
cable delivered but has wrong plug, build catladder and platform or investigate moving

13. Make sure all expats travelling to site have did PCR tests in advance and that the site
manager has been made aware, see email sent to you
