    Nonconformance Report: Internal External Survey

    This nonconformance report documents an issue with the gasket face of a nozzle (N4) on a heat exchanger that was damaged after heat treatment. An observation memo was also raised notifying that the gasket face was not protected when moving the heat exchanger between sites. To address this, a corrective action report (TA-CA-007) was issued to the production department requiring a preventive action plan to avoid repetition of the problem. The root cause is to be analyzed and the proposed corrective actions are to be approved by management before verifying the actions have been completed satisfactorily.

    The gasket face for nozzle N4 is found damaged after heat treatment. Site observation memo (SOM 14-001) was also
    raised to notify production depatment for not protecting the gasket face while moving the heat exchanger from TSM
    ARABIA site to SSP furnance.In order to avoid repetetion of this problem, CAR (TA-CA-007) is raised and production
    dept. is required to prepare a preventive action plan to eliminate its re-occurance.
