Sub: Disconnection of my BSNL Broadband connection

Sir,

With reference to the subject mentioned above, I, Mr. ________ would like to
disconnect my BSNL Broadband connection due to unreliable service and frequent
disconnection.

I kindly request you to take the necessary action to complete the Broadband
disconnection process at the earliest. All pending bills have been paid and there is no
outstanding amount nor am I in possession of any BSNL rented Router or Modem.

Please further note that I only wish to disconnect my broadband connection and not my
phone line (landline). The details of my account are as follows.
