HISTORY

Cristo Rey is one the twenty [20] barangays of the Municipality of Capas, Province

of Tarlac. It is formerly known as O’Donnell Resettlement Center. However, this place

was originally known as Camp O’Donnell, the consecration camp of the Filipino-

American Soldiers during the infamous Death March. In 1947 it became a military base

of the United States of America [US Naval Radio Transmitting Facility]. Due to the

power of Executive Order 813 this was converted to a resettlement site by the late

president Corazon C. Aquino, a place where the victims of Mt. Pinatubo from the hard hit

towns of Conception,Bamban and Capas were relocated. The renaming of this barangay

from O’Donnell Resettlement to Brgy. Cristo Rey was unanimously approved by the

Sangguniang Panlalawigan of the Province of Tarlac by the Provincial Ordinance No.

004-2003 0n August 7, 2003, an ordinance creating O’Donnell Resettlement area in

Brgy. Aranguren an independent barangay.

CRISTO REY HIGH SCHOOL formerly known as O’Donnell Resettlement

High School has been in existence since 1994 to cater to the educational needs of the

victims of Mt. Pinatubo’s eruption from the hard hit towns of Capas, Conception and

Bamban. On March 8, 2005, O’Donnell Resettlement High School was altered to Cristo

Rey High School after the unveiling and renaming of all schools in the resettlement.

The following became the administrators of the school: Mr. Samuel R. Serquina,

Teacher In-Charge, Dr. Estrella C. Honrado, Mrs. Racquel G. Cayabyab, Mrs. Ester

Medina[2002-2004] , Mr. Ernesto Cunanan, Mr. Mario M. Tayag [2006-2010], Dr.

Epifania B. Dungca, Mrs. Teofista A. Dantes, Dr. Noel L. Linsao, OIC, Dr. Arlene P.

Apostol (2016 to present).


INTRODUCTION

When I knew that I will be assigned at the Principal’s Office at the first I was

uneasy then I realized it would be an interesting task for me as an OJT. At the first I was

wary to socialize w/ Mam Lian but later on I enjoyed her company and I learned so much

from her. I’ve learned how to properly file documents, arrange 201 files and

communicate to teachers and especially to our Principal. I will treasure all their teaching

and advised to me and to apply them in my daily life.

DAY 1 – January 10, 2018

My first day in the Principal’s Office is about orientation of what to do and not to do in

the office. I was also introduced to our Principal Dr. Arlene P. Apostol and Ma’am Lian.

She just let me observe her on her daily tasks so that I will familiarize myself with the

office and with the documents.

DAY 2 – January 12, 2018

On my 2nd day, Ma’am Lian taught us how to summarize the test banks that are

submitted by department. We drafted it first on a clean scratch paper before encoding it.

We summarized it by department so that we will be able to see who among the

department have submitted on time. It was a little bit confusing but because we are

supervised by mam Lian, we did our task well.

DAY 3 – January 15, 2018

We encoded NOSA (Notice of Salary Adjustment) of the Senior High School Teachers.

At first, it was tiring because we should always be cognizant of the data that we are

encoding because there should not be any error. It was fun because we learned a lot from

doing this.
DAY 4 – January 16, 2018

My task for this day was to record and keep track on the Daily Lesson Log of the

different departments. They are usually submitted every Mondays. We put on dates and

record them on the Log book for future purposes and for an updated record management.

We assist Mam Arlene when she signs the DLL and afterwards, we return the DLL on the

respective department heads.

DAY 5 – January 17, 2018

We are assigned to update the bulletin board and deliver documents and letters to

different teachers and head teachers. It made us be comfortable with the head teachers

and teachers.

DAY 6 – January 18, 2018

We prepared the DLL again for signing of Mam Arlene. We also received documents like

leave forms that are for signing. Mam Liann also asked us to arrange the documents of

the file tray of per department. We encoded and printed the reports and documents

submitted by the head teachers.

DAY 7 – Janaury 22, 2018

On this day, we filed documents and arranged them by order. It is very important that our

files are arranged and always updated.

DAY 8 – January 23, 2018

On my 8th day my task is to organize the files and cleanliness of the office. I am happy

that Ma’am Lian and Ma’am Principal are satisfied with the organization and cleanliness

at office.
DAY 9 – January 29, 2018

It is my 9th day as an OJT and I am experiencing mixed emotions because I’m happy that

I was able to survive my work immersion, at the same time sad in a way that I will leave

Ma’am Lian and Ma’am Principal because I already enjoyed their company.

DAY 10 – January 30, 2018

My Last day I was given the task to organize the leave forms of teachers. We arranged

them by month and by Department. I was quite sad because it is my last day of work

immersion I will surely miss Ma’am: Lian and Dr. Arlene Apostol. I am glad that I had

the opportunity to be working with them.

Behavioral Expectations

BE RESPECTFUL

1.Pagdalo sa flag ceremony o flag retreat at buong galang na pag –awit ng pambansang

awit o Lupang Hinirang /Awit ng Rehiyon 3/Awit ng Tarlac Awit ng Capas at pagbigkas

ng buong linaw at may paggalang sa Panatang Makabayan at Panunumpa sa Watawat

(DepEd Order No.86,2010).

2. Pagbibigay ng respeto sa kapwa mag –aaral sa mga guro att awtoridad ng paaralan sa

lahat ng oras.(D.O.No.02 s.1992).

3. Hindi sa pagkuha sa anumang gamit na hindi pag-aari .Ang anumang gamit ng kapwa

mag-aaral o nh paaralan na nais hiramin ay ipagpaalam bago gamitin at ibalik ng maayos

sa pinag hiraman o kinauukulan (DepEd Order No.92 s,1992)

4. Hindi kalianman mananakit o manakot o mamumuwersa ng kapwa mag-aaral sa kahit

na anumang paraan.(Anti-Bullying Act.)

5. Hindi maninira ng mga opisyal ng dokumento ng paaralan tulad ng I.D.,card Form 137

atbp.(D.O.92 s.1992)

6. Iingatan,igagalang,at pananatilihing maayos ang mga gamit at istruktura ng paaralan

(D.O.92, s.1992)

7. Pag-iwas sa pagbibiro at pagbibitaw ng mga salitang makasasakit o makasasama sa

kapwa mag –aaral lalung lalo sa mga guro at kinauukulan ( D.O.92.s.1992)

8.Pag-iwas makalikha ng anumang ingay na makakaistorbo sa mga kapwa mag-aaral o

iba pang nagkaklase


BE SAFE

1.Pananatili sa paaralan tuwing oras ng klase.Hindi paglabas sa paaralan sa pamamagitan

ng pag-akyat sa bakod o pag-o-ober da bakod.kung kina-kailangan talagang lumabas

,kumuha ng pass slip at ipapapirma sa guro,adviser at Grade Level Chairman.

2. Pag- iwas sa paninigarilyo ,pag inom ng alak,pagagamit ng ipinag babawal na gamut o

maging pagdadala lamang ng mga ito.(DepEd. Order No.37 s. 2010)

3. Pag –iwas sa pagdadala ng anumang bagay na makapipinsala sa kapwa tao .(deadly

weapon,tear,gas,explosive,pornographic materialsatbp.)

4. Pag iwas na mapabiling o matatag ng anumang iligal na organisasyon o grupo na hindi

inaprubahan ng paaralan na maninira sa katahimikan at kapakanan ng paaralan at mga

mag-aaral (DepEd Order No. 92 s.2009-Prohibittion on the Practice of Hazing

&Fraternity/Sorority)’

5. Pag-iwas sa pagsususgal o pakikipag pustahan s aloob o labas man ng paaralan.

6. Hindi pagdala ng anumang kagamitan hidni sa karaniwang ginagamit sa oras ng klase

upang maiwasan mna ito ay masira o mawala

7. Pag-iwas sa pagsusuot ng mga palamuti sa katawan na hindi kaaya-aya at hindi sa

modelong mag aaral

8. Pag-iwas sa pagpunta sa mga computer shop at maglaro ng computer games sa tuwing

oras ng klase

9. Pag-iwas sa pagtangkilik sa gawaing”Public Display of Affection”.

BE RESPONSIBLE

1. Pagpasok sa paaralan araw-araw at pag-iwas sa pagliban ng klse ng walang sapat na

dahilan .

2. Pagpasok ng tamang oras na itinakda ng paaralan.

3. Pag-iwas sa pagka -cutting classes o pag e-escape.(D.O.92,s.1992)

4. Pagsuot ng uniprme na itinakda ng paaralan at I.D na may akmang lace.


5.Pagpapanatiling maiksi ang buhok(lalaki-isang pulgada mula dulo ng itaas ng tenga at

tatlong oulgada mula sa kwelyo ,maayos at walang kulay (lalaki at babae)

6. Pag-iwas sa pangongopya at pandaraya sa panahon ng pag susulit

7. Pag bibigay alam sa mga magulang o legal gruadian sa tuwing may pagpupulong at

nay iba pang tagubilin ang mga guro .

8. Pag-iwas ng paggamit ng cellphone sa oras ng klase at pagtitext ng nakakasakit sa

kapwa kamag aral o maging sa awtoridad ng paaralan (DepEd Order No. 83 S. 2003-

Prohibiting Students from Using Cellural Phones during Class Hours)

9. Pagpapanatiling maayos at malinis ang sarili at pag iwas na gumamit ng kolorete sa

mukha maliban kung bunga ng gawaing pampaaral at may kapahintulutan ng guro.

10. Pagtapon ng basura sa tamang lalagay at pagtulong sa pagpapanatili sa kalinisan at

kaayusan ng paarsalan at kapiligiran. Upang mahasa sa tama at mabuting disiplina ang

mga mag aaral ang bawat paglabag sa patakaran ay may kaakibat na kaparusahan .Para sa

minor offenses,ang kaparusahan ay ang mga sumusunod.

All teaching and non-teaching personnel of Cristo Rey High School have legal
