Dear Sir/ Madam,

Re: Mr Alfred Billy, aged 52

I am writing to refer Mr Alfred, who was diagnosed with skin cancer on his neck.
After recovering from his operation, he is now ready to be discharged and requires
your assistance and support for home care.

During the stay at our hospital, Mr Alfred had skin biopsy done, which showed two
types of skin cancer, basal cell carcinoma and nodular basal-cell carcinoma. Following
the removal surgery, he has been placed on 500mg of Panadein Forte for surgical
pain, with good effect. He has made good progress since.

In terms of Mr Alfred’s social history, he is a truck driver with habits of drinking and
smoking. He lives with his wife, Maria Jennifer, who is also his emergency contact.

Please note that Mr Alfred will need to take his pain medication for one more week
after his discharge. Also, his observations need to be taken daily.
