(very limited stocks on supplier!!)

The above proposal is subject to the following Terms and Conditions
1. Other defective parts, if any, will only be determined after replacing the above-mentioned parts.
2. No warranty on consumable parts such as Printer Head, etc. Software is not covered by warranty.
3. Extends (30) days warranty on repair done. Warranty does not cover defects (hidden) that are not evident at the
time of inspection/diagnosis. The replacement part/s will have (3) months warranty after receipt of the machine
from Microphase.
4. Equipment not claimed within 60 days after the completion or rejection date shall be sold to cover the cost of repair and
storage charges.
5. No response within 15 days from receipt of quotation would mean non-conformance, and storage fee of P 50.00/day shall
be charged immediately after the 30-day grace period.
6. Please signify acceptance by signing below this proposal under “Terms and Conditions accepted:” and refax the same
to MICROPHASE.
7. Prices are subject to change without prior notice. This quotation is valid only for seven (7) working days.
8. Terms: 30 DAYS
9. Note: If you find the foregoing in order please sign on the space provided below and send back to us at Fax No.:
8180615/7500825/8140723. Customer may discontinue repair authorization after receipt of quotation and agrees to pay
for the consultation fee equivalent P 1,250.00. We hope the above proposal merits your favorable consideration. For any
