COMPLIANCE OF
REQUIREMENTS

requirements during duty (RLE) days.

• Failure to attend orientation,


evaluation, and scheduled
conference/ward class.

UNIFORM

• The student nurse must be well


groomed at all times.

Failure means 1 day RLE


extension.
JEWELRY

• Student are prohibited from


wearing jewelries.

• Any CI may confiscate the


jewelry worn by the student.
PERSONAL GROOMING
• All students are expected to adhere
to proper cleanliness and good
grooming.
• Ladies: wear light lipstick and face
powder.
Fingernails must be short, clean,
and with no colored nail polish.
Long hair kept up in a bun.
PERSONAL GROOMING
• Men will have their haircut maintained
in a manner acceptable under the
general criterion “clean cut.”
• No mustache nor beard.
• Shoes must be clean and in order.
No sandals, no slippers.

Failure to comply means 1 day RLE


extension.
CLINICAL DISCIPLINE
• Procedure
1. Any offense or violation of the
rules and regulations should be
reported in writing.

2. For any major or minor offense,


the student is required to submit
four (3) copies of written explanation
(Incident Report) w/in 24 hours.
• (1 for the CI, 1 for the Dean, 1 for each
level coordinator (clinical, academic)
relating the facts of the incident.

• Should be duly countersigned by the CI


concerned or her/his representative.

3. For any major or minor offense,


the case will have to be elevated to the
Committee on Standards.
OFFENSES
• Major Offenses
a. Gross disrespect or gross
discourtesy to CI, Hospital Personnel,
peers and visitors.
b. Gross dishonesty as
b.1 Deliberate failure to report an error
in medication or procedure.
b.2 Cheating
b.3 Forgery and tampering of clinical
records & student’s records.
b.4 Stealing
MAJOR OFFENSES
c. Assaulting or inflicting harm to any
person in the clinical area where the
student is assigned.
d. Drunkenness while on duty.
e. Carrying of deadly weapon while on
duty.
f. Use or possession of prohibited
drugs
g. Gambling in any form while on duty.
Major Offenses
h. Vandalism in the hospital, clinical,
CHN areas such as:
 breaking of glass window door &
cabinets
 writing, drawing and carving on the
walls and furniture
 tampering with official notices and
posters or,
 unauthorized removal of the same.
Major Offenses

i. Revealing to unauthorized persons


information that tends to reflect
upon the condition and moral
character of the patient.

j. Arrogance in the clinical areas.


SANCTIONS
• Extension of duty in the clinical
area/community where the offense is
made to be determined by the
Committee on Standards of Nursing.

• Repetition of clinical experience/CHN


requirements.
SANCTIONS
• Suspension of clinical/CHN
experience.

• Other measures which will be


imposed depending on the
severity of the infractions and or
offense.
MINOR OFFENSES
• Watching television in the patient’s
room or other areas while on duty.

• Smoking in the hospital premises


(including hospital canteen).

• Shouting, giggling, screaming in the


hospital.
MINOR OFFENSES

• Reading of newspaper or magazine


while on duty.

• Eating while on duty not within their


scheduled snacks and mealtime.

• Using of health care facilities


without C.I.’s permission.
SANCTIONS
• Warning the student that repetition
of such act may be a cause for other
disciplinary measures.
• Oral or written apology by the
students addressed to the offended
party if necessary.

• Extension for one (1) RLE day for


every offense.
GENERAL RULES AND
REGULATIONS
• Students are allowed to have their coffee
break for 15 minutes and a lunch/dinner break
for 30 minutes.

• Avoid over familiarity with doctors, nurses


and nursing attendants. Use their family
names when addressing.

• Students are required to be in their smock


gown, when they are in the hospital premises
outside of duty hour.
ABSENCES

• Maximum of 3 absences

beyond 3 means A/F
