AFFIDAVIT OF LOSS
I, ASISILO G. VILBAR, of legal age, married, Filipino, and a resident of Purok
6,South Poblacion, Maramag, Bukidnon, after having been duly sworn to an oath in accordance with law, hereby depose and say:
1. That I am the duly authorized representative of my brother, Herminio G. Vilbar,
the registered owner of a parcel of land situated in the Barangay South poblacion, Maramag, Bukidnon, with Transfer Certificate of Title No. T- 30152;
2. That the Registry of Deeds for the Province of Bukidnon issued an Owner’s Duplicate of Transfer Certificate of Title No. T-30152and was under my custody for safekeeping particularly described as follows;
A PARCEL OF LAND (Lot 2-A, Psd-10-014008 being a portion of Lot
2, Psd-10-011992) situated in (the Pob. Mun. of Valencia, Prov of Bukidnon, Island Mindanao. Bounded on the NW, along lines 4-5-1 by Lot 2-B( Right of Way 3.00 m. wide) on the N. along line 1-2 by Lot 2-C both the subd. Plan Psd-10-014008, on the SE., along line 2-3 by Lot 313 Cad- 791-D Valencia Cad. On the S. along line 3-4 by Road.
Beginning at a point marked “1” on the plan, being S. 62 deg. 24’W.,
555.34m from BLLM No. 1, Gss-370.
Thence S. 89 deg. 11’E., 9.08 m. to point 2:
Thence S. 9 deg. 03’W., 30.00 m. to point 3; Thence N. 89 deg. 09’W., 9.03 m. to point 4; Thence N. 8 deg. 52’E., 25.00 m. to point 5; Thence N. 9 deg. 27’E., 5.00 m. to point of
Beginning; containing an area of TWO HUNDRED SEVENTY (270) square meters
more or less. All points referred to are indicated on the plan and marked on the ground as follows: Point 3 by Old points, and the rest by PS cyl. Conc. Mons 15 x 40 cn. Bearing true, date of original survey June 30, 1975, and that of the subd.. June 22, 1990, executed by Felino S. delos Reyes, Geodetic Engineer and approved on October 8, 1990.
3. That sometime in the February 2019, I misplaced the said Owner’s Duplicate of Transfer Certificate of Title;
4. That all efforts were exerted to look for the said Owner’s Duplicate of Transfer Certificate of Title, but it proved futile and it is beyond recovery and is deemed lost for all intent and purpose;
5. That I am attesting to the truth and veracity of the foregoing facts;
6. That I am voluntarily executing this affidavit to file a petition for re-issuance of title and for whatever legal purpose/s it may serve.
