The following outlines the detailed Scope of Work associated with the Contract.

2.6.1 Construction Sequence
The Principal will supply a proposed program to the Contractor.
2.6.2 Program Key Dates
The key dates for the Contract are as follows:

* The Contractor shall ensure that their contract management team and an agreed portion of their direct labour
workforce have fully completed the induction process and are ready to commence the Works on the mobilisation
date.

2.7 Contract
or and Principal Responsibilities
The Contractor shall provide the whole of the Works which are described or implied in the Specification.
All works omitted from this document, that may be inferred to be obviously necessary for the completion of the
Works and the satisfactory performance of the equipment to be supplied, shall be deemed to be included in the
Schedule of Rates.
Works not shown on the Contract Drawings but mentioned or described in the SoW shall be deemed to be
included in this Contract.
The Contract Drawings that form part of this SoW, provided at the time of Tender were for Tender submission
purposes only and any omissions or inaccuracies will not be reason for Contract variations or extension of time.
All work included in the Contract shall be supervised at all times during the Contract by a qualified representative
of the Contractor and full facilities and assistance shall be given by the Contractor to the Principal to check the
Works.
2.8 Site
Conditions
2.8.1 Site Location and Access
The Lead Smelter is situated on MIM lease. Access is via Gardenia gate for vehicle access

Drawing Records
The Contractor shall ensure that as installation work proceeds, an accurate “As Built” of the installation is
maintained. The Contractor shall reserve one set of drawings as a “master mark up”.
Drawings shall be marked up and filed as described in “Appendix 1 - Drawing Mark Up Procedure. Subject to the
Principal’s written approval, the Contractor may use a different procedure providing it is more rigorous than that
given herein.
A copy of every drawing issued to the Contractor to carry out the Works shall be returned to the Principal certified
“As Built”.
2.11 Cleanin
g on Completion
On completion of the Works, the Contractor shall take down and clear away all rubbish, waste, plant and
temporary work, including sanitary conveniences, offices, sheds, and hoardings, protective covers etc unless
otherwise required and make good the site to the satisfaction of the Principal. Any damage done to the buildings,
structures, plant, equipment or property shall be made good at the Contractor’s expense.
The Contractor shall clean the complete Works including but not limited to the work described below;
a. Wash, if necessary, to clean floors, both inside and out on all faces, etc, paying special attention to
removing mud, dust, dirt and debris from inside the Switchroom and control room.
b. Remove all paint, grease, and building marks on all ceilings, walls and floors, metal surfaces, sections,
etc.
c. Thoroughly clean all substation plant & equipment, vacuum internals of Switchgear, Control Cabinets,
Junction Boxes and the like.
Any oil and chemical spills shall be contained and disposed of in a manner consistent with the Principal’s
environmental protection requirements.

Contract
Any Contract arising from this Scope of Work shall be ‘Schedule of Rates’ in accordance with the Mount Isa Mines
Pre-qualification Agreement and Standard Conditions of Contract.

4.4 Principa
l’s Representative
The Principal’s Representative is listed in the Particulars of the General Conditions of Contract. All project
correspondence shall be directed to the attention of the same.
4.5 Site
Inspection
The Contractor shall be deemed to have examined and inspected the site and its surroundings and had satisfied
themselves, before submitting the tender, as regards to the existing ground condition, roads or other means of
communication with an access to the site, the form and nature of the site, the risk of injury or damage to property,
the nature of materials to be lifted, the nature of the Works and materials necessary for execution of the Works,
and generally to have obtained their own information on matters affecting their tender and the execution of the
Works.
No claim by the Contractor for additional payment shall be allowed on the ground of any misunderstanding in
respect of the matters referred to in this document, or otherwise on the ground of any allegation or fact that is
incorrect, or on the grounds that insufficient information was given by any person whether in the employ of the
Principal or not, or of the failure of the Contractor to obtain correct and sufficient information, nor shall the
Contractor be relieved from any risk(s) or obligation(s) imposed on, or undertaken by them under the Contract on
any ground or on the ground that they did not or could not foresee any matter which may in fact affect or have
affected the execution of the Works.
4.7 Tender
Submission Requirements
The Tenderer shall provide fully detailed and complete Tender schedules and compliance statements as part of
their Tender submission.
The Tenderer shall submit a preliminary program to that outlines the delivery of the Works. This is required to
satisfy the Principal of the Contractor’s ability to finalise the work in the allocated time.
The Tenderer shall submit examples of similar projects previously undertaken by the Contractor shall be provided.
Evidence shall be provided of successful implementation techniques and strategies to address the requirements
of this Scope of Work.
The Tenderer shall provide a proposed organisational structure for the key personnel that WILL BE ACTIVELY
WORKING on the project if successful in the tendering phase. Previous work experience and qualifications shall
be provided for all personnel.
The Tenderer shall submit a statement of resource detailing the resources, labour, plant and equipment it
currently has available to demonstrate its ability to complete the Works should their offer be successful.
5.2 Roster
The Contractor’s personnel are required to work a roster with a minimum of 13 days on site per rostered period.
The roster shall consist of permanent day shift and a night shift as required for all onsite activities on the Contract
critical path. Should a night shift be required, the Contractor shall alternate their work crews each roster period.
5.3 Hours of
Work
Each shift for the Contractors underground workforce shall be 12 hours per day commencing at 6:00am/pm.
5.4 Supervi
sion
The Contractor shall be required to provide full time supervision for the duration of the Works.
The Contractor’s supervision shall report to the Contractor’s site manager who will report to the Principal’s
Representative.
5.5 Senior
First Aid
As a minimum at least one employee of each crew that enters the site shall have a current senior first aid
certificate. The Principal will request documentation/copies of relevant certificates prior to commencement of the
Works.
5.6 Contract
or Requirements
When working in any part of the plant or lease all Contractors, Sub-Contractors and their personnel shall abide by:
All personnel requiring access to site shall have current:
a. Metalliferous Mines Generic Induction (surface)
b. Fire and CPR certificate
c. )
d. Height Safety course (for personnel required to work at heights)
e. Confined Space Entry course (form personnel required to work in confined spaces)
The Contractor shall be required to submit a record of current Blood Lead (PbB) levels for all project personnel
requiring access to site, prior to commencement (refer Project Personnel Register). This may require that all site
personnel undertake a blood test prior to commencement on this project.
5.7 Site
Access
Access to site for personnel must be obtained as per Mount Isa Mines Procedure PRO-120000 Site Access and
Security – Workers and Visitors.
Access to site for vehicles and mobile equipment must be obtained as per Mount Isa Mines Procedure PRO-
120100 Site Access and Security – Vehicles & Mobile Equipment. Prior notice of one week shall be given to the
Principal for organising site compliance.
The Contractor shall be required to submit a Contractor Access Application FRM-120002 to the Principal’s
Representative at least two working days prior to commencing on-site work.
5.8 Hazard
Identification, Risk Assessment and Control
The Contractor shall actively identify and remove/control hazards in the workplace to create a safe work area for
the entire Contract period. If the Contractor is aware of a hazard/s and does not have the ability to remove the
hazard/s, then the Contractor shall report the hazard/s to the Principal’s Representative.
Work shall not start or continue until hazard/s are identified and isolated from the work area.
As part of the Job Safety Analysis, the Contractor shall be required to document the formal process of Hazard
Identification and Risk Assessment in accordance with the MIM pro-forma attached to SH&E standard.
This process shall be completed in conjunction with the Project Personnel.
5.9 Job
Safety Analysis
A detailed Job Safety analysis (JSA) shall be submitted for review by the Principal’s Representative prior to the
commencement of site work. The JSA shall address items including personal protective equipment, height safety,
lifting practices, isolation and tagging, equipment tagging, emergency procedures, tool box talks, site
communication, reporting of incidents / accidents, first aid procedures and equipment operation.
All persons associated with the site works shall be inducted into the JSA, by reading, understanding and signing-
off on the aforesaid documents by way of an induction log prior to commencement on site. All new persons
entering the site works after commencement of the site works shall also be inducted into the JSA in this same
manner.
The induction log and current versions of the JSA shall be kept on site at all times. A current copy of the JSA shall
also be available on site for reference and update purposes at all times.
Activities not specifically covered by an authorised JSA shall not be commenced. Any modifications to the JSA
(including additions of new activities) to be reviewed by the Contractors supervisor and the Principal’s
Representative prior to commencement of activities.
The Contractor shall ensure the JSA is continually audited and amended as necessary to optimise the safety and
efficiency of the Works. The Contractor shall maintain its own Safe Work Instructions and the Contractor's
employees shall comply with these procedures.
5.10 Shift
Pre-Start Meetings
The Contractor shall perform a pre-start meeting with its personnel at the beginning of each shift. The pre-start
meeting shall encompass attendance, safety, handover information from the previous shift, planned tasks for the
shift and any other relevant information or topic. The Contractor shall keep a record of the pre-start meeting.
Contractor pre-start meetings will be attended by the Principals Representative or his delegate.
5.11 Work
Permits
The Contractor shall not commence any works without first obtaining the appropriate work permits from the
Principals Representative for that shift. The work permits may include the following;

6.1 Tooling
The following items are deemed as standard industry tooling and shall be supplied by the Contractor.
6.2 Persona
l Protective Equipment (PPE)
The following items are deemed as standard industry PPE and shall be supplied by the Contractor. The supply of
such PPE shall be allowed for the Contractors charge out rate;
a. Safety capped work boots (lace up)
b. Safety capped gum boots
c. Hard hat/safety helmet
d. Safety glasses (dark & clear)
e. Safety goggles
f. Face shields
g. Respirator
h. Respirator carry bag
i. Replacement respirator filters & cartridges
j. Gloves
k. Safety harnesses and lanyards
l. Water bottles
m. Any other standard piece of PPE required by the Contractor to safely carry out the Works.
6.3 Mobile
Plant and Equipment
The Contractor will be required to supply and maintain the following mobile plant and equipment, chargeable to
the Principal on day rates or part thereof per the Contractors schedule of rates;

6.3.2 General
The Contractor shall ensure that all mobile plant equipment complies with the relevant site standards and is
maintained in safe mechanical and operational condition in accordance to the manufacturer’s specifications.
The Contractor shall program the maintenance of its mobile plant and equipment as to not affect the continuity of
the Works. Should this not be possible the Contractor shall arrange alternative plant for duration of the
maintenance.
7.2 Facilitie
s
The Principal will supply the following surface amenities for use by the Contractor during the Contract;
a. Crib Room
b. Ablutions
7.3 General
The Contractor acknowledges that the drawings in the drawing schedule is not an exhaustive list of all drawings
that may be required to complete the Works. Additional drawings, data sheets, manuals ect required to complete
the Works under the Contract will be made available to the Contractor upon request.
The works shall comply with the requirements set out in the Drawings listed in drawing Schedule.
