1. Write the types of industries that required safety and health officer, according to SHO
Order.
2. Delicious Pastries Sdn Bhd, has been directed by the Director General of Department
of Occupational Safety and Health to have safety and health committee [SHC] at their
workplace.

a. Establish the SHC [in term number of members and the composition] for
Delicious Pastries Sdn Bhd which has total number of worker of 185.

o Chairman (1)
o Don’t need Secretary @ safety n health committee
 According to safety n health officer order for manufacturing (j)
use safety competent if worker more than 500

- Osha 1994 (safety n health officer order)

b. Explain the reason for your answer in 3a according to statutory law(s) (Act /
Regulation/ Order/ Code of Practices/ Guidelines; whichever applicable).

3. During the routine inspection of DOSH officer to a petrochemical plant of 250 staff, Excell
Sdn Bhd; the officer had instructed a new safety and health committee (SHC) to be
reestablished. If you were the DOSH officer, point out the UNCOMPLIANCES according
to regulation {state the reason and name of regulation that has been referred}. Details of
the present SHC at Excell Sdn Bhd as listed in Table 2.

corridor for the protection and prevention against fire.

a. Determine the types and quantity of fire extinguisher required along the corridor
for the EXCELL HOLDING NEW ADMINISTRATION BUILDING.
b. State the reason for selecting of each type of fire extinguisher at certain location.
