Test 1

The document describes a chemical reactor system and provides an event tree analysis for a loss of cooling initiating event. It then lists the failure rates of different safety functions. It asks to calculate the probability of plant shutdown and process runaway per year. It then describes a chemical reaction process involving liquids A and B flowing into a stirred reactor to produce product C. It asks to perform a HAZOP study using the guide words no, more, and less on the process parameter of flow for study notes 1, 2, and 3.

Question 1

Consider a chemical reactor system in the above figure. The system is equipped with a high temperature alarm to warn the operator
of a high temperature within the reactor. An Event Tree Analysis for this system using the “loss of cooling” as a Iniating Event has
been developed as the following. Determine the probability of plant shutdown and process runaway per year with the failure rate
of each safety function as stated. From the data shown, it is expected the event Loss of cooling occured four times a year.
Safety Function
Hot Water Operator Notice Operator Operator
Result
Safety function fail High Temperature re-start cooling Shut Down reactor
Failure rate 0.1 0.5 0.1 0.25

Question 2
The chemical reaction involves 2 liquids, A & B, which flow from their respective storage tanks into the stirred product C reactor.
The flow rate of materials A & B is regulated by control valves A & B, respectively. The liquid phase chemical reaction that results
is highly exothermic & generates considerable quantities of heat, which must be controlled by cooling water on the jacket of
product C reactor. The reaction of materials A & B to produce product C proceeds rapidly, & product C is drawn off from the bottom
of the reactor. Product C flow through valve C into the product C storage vessel to await shipment. The reactor & each of the
storage tanks have pressure-operated relief valves in case of pressure buildup during the process operation.

Carry out HAZOP study using OSHA guide words of No, More and Less for the process parameter FLOW for study note 1, 2 and
3 using HAZOP worksheet.
