Dear Respondent,
The purpose of this survey is to explore the effect of balanced scorecard on psychological
empowerment and managerial performance. This questionnaire has been designed to
assess your perception regarding use of performance appraisal through balanced scorecard
and its effect on psychological empowerment and managerial performance.
There are four sections in this questionnaire. “Section-A” aims to identify the demographic
information of the respondents. “Section-B” is designed to know the information regarding
balanced scorecard, “Section-C” is designed to know the information regarding
psychological empowerment and finally “Section-D” is designed to know the information
regarding managerial performance. Each question will be rated as seven (5) Point Likert
Scale, where One (1) represents strongly disagree and the five (5) shows strongly agree
with the particular statement.
