1. PURPOSE
This document defines the method to be adopted to ensure the Formworks installations that are
conducted in the site are in acquiescence with the Structural Works Technical Specification,
approved Materials and Approved for Construction Drawings. Where the materials to be install are in
compliant with the approved design and using recognized/standard installation practices that are
safe and in accordance with the Project Safety Plan and associated safety procedures. To ensure
the work procedures are complied with the Project Quality Plan, its associated quality procedures
and an approved Inspection and Test Plan for the works.
The method statement describes the procedure for Material Delivery Inspection, Fabrication and
Installation of FORMS.
This method statement also focuses within site materials inspection, fabrication and installation
method to ensure that the FORMS installed, and inspected is in accordance with project
requirements. The work method is summarized as follows:

5. SPECIFICATION CLAUSE
The specification requirements for FORMS are described within Project Technical Specification of
Structural Works and For Construction Design Drawings as follows:
All materials to be used shall be approved by the designer and comply as required on the above
stated for construction drawings.

6. RESOURCES AND MOBILIZATION
According to the work program and planned site access availability, appropriate staff, labors,
equipment and materials will be mobilized to the site and install FORMS with the approved “For
Construction Drawing and Approved shop drawings issued by EELHI and approved by the Structural
Works Design Consultant.
The actual mobilization of staff, workers, equipment and the materials delivery schedule will be
dependent on site accessibility. Actual site access will need to be notified by NGCB at least 2 weeks
before the actual execution of the works.
It is understood that NGCB will be responsible for the provision of appropriate type of crane for the
transportation of materials and equipment to complete the works including temporary construction
water and power supplies and lighting system.
7. CONSTRUCTION EQUIPMENT
It is envisaged that the tower crane will be required by the NEW GOLDEN CITY BUILDERS for the
activities involved in the supply and installation of Formworks. The typical construction equipment
requirements are as listed below and will be used up to the completion of the activities.
8. PERSONNEL
Formworks installation will be organized from central location by Project Construction Team, both
General Contractor and Nominated Subcontractor. It is envisaged that the installation will be
conducted by the followings:
Formworks Engineer
Survey Team
Quality Control Engineer
Safety Officer
Foreman
Tower Crane operator
Carpenters
The above mentioned personnel shall be fully competent and trained to carry out all works
associated with the Formworks installation. They shall have attended the required contractor’s safety
induction and will abide by the Health and Safety Plan, policy and procedures as well as
Environmental plan.
9. MATERIALS
FORMS should be fabricated at the site fabrication area (except for TABLE FORMS, COLUMN
FORMS, and BEAM BOTTOMS & SIDINGS. The said items will be fabricated at NGCB
WAREHOUSE Napindan, Taguig) according to the site requirements and in accordance with the
approved “For Construction Drawings and/or Approved Shop drawings”. Forms shall be inspected at
the fabrication area before installation at site.
The unloading of Forms like Table forms, Column forms, one side forms and metal panel will be
carried out at site using a tower crane provided by NGCB.

All materials delivered to the site will be inspected by EELHI Engineers In-charge, to check that it is
in accordance with the site requirement and conform to the approved design and material
qualification. Delivery of FORMS shall be inspected. Rejected delivery of forms shall not be accepted
and shall be returned.
The inward inspection of materials will be carried out in accordance with the relevant of a QA/QC
procedures and or Quality Plans approved by EELHI.
EELHI Engineers has the authority to perform any inspection for the storage of materials from his
own point of view for the checking of material quality or the quantity to maintain the quality
compliance and sufficiency of materials in the site.
Note: No materials shall be permitted to enter and/or installed inside the jobsite without
inspection and approval from CQC & EELHI Engineers upon arrival at jobsite.
The construction procedures and methods described herein are based upon the scope and
application of the method statement summarized in section 2 of this method statement and the work
methods are organized accordingly.
Before the commencement of installation of Formworks the following are required to be carried out:
Check the approved design and drawings for forms are issued with Approved for
Construction status, and in accordance with the document control procedure.
Check the site access availability for the work is, clean and safe.
Check the forms and other miscellaneous materials used for installation of formworks have
been ordered and inspected at NGCB fabrication area and available/ready for use in the
site.
All delivered materials shall be inspected upon arrival at the site. The inspection shall be
advised, in advance, as required by the ITP.
The delivered materials shall be inspected in accordance with the appropriate Quality
Plans and Procedures.
The delivered materials shall be in accordance with the approved design technical
specification, approved for construction drawings and approved material submittal.
Material approvals in form of approved SRF and all applicable documentations and
certifications shall be available during incoming material inspection.
The materials shall be free from defects or damaged.
Any materials delivered in the site that are not complying with the approved material
submittals and structural technical specification or rejected by EELHI Engineer shall either
be returned to the point of supply or stored in the site at a suitable quarantine store. NCR
shall be raised for all such materials.
Generally, prior to commencement of the delivery of the formworks materials, the setting
out of the installation shall be conducted to determine the location and elevation.
The setting out of the works shall be on the basis of an “approved for construction
drawings and or approved shop drawings” for setting out. Settings points shall be from the
common reference point established provided in the site by NGCB.

Any discrepancy between the common reference point on the approved for construction
drawings and the actual site provisions shall be raised thru RFI’s by NGCB subject for
Consultant’s final evaluation.
Check and review approved SRF and for Construction plan layout.
Secure the signed/approved I.R of Rebar, and MEPF if needed and attached in the FER
for submission to the EELHI before the commencement of the works.
Establish reference GL (gridline) & EL (elevation) for laying-out.
Double check lay-out and dimension of Shear wall, Columns, Retaining wall and RC wall
before the installation of forms.
Check and install MEPF embedments or block-outs as per MEPF specification and/or
FCD.
Set forms mortar-tight and true to line and grade.
Coat the contact surfaces of the forms with form oil. Do not use mineral oil on forms of
surfaces to which adhesive, paint, or other finished material is to be applied.
For structures exposed to earth, water and moist, provide bentonite strip at every
construction joint and polyethylene sheets for vapor barrier.
Provide formworks clean-out openings to permit inspection and removal of debris.
Make sure that adequate number of spacer blocks installed and required concrete
covers are attained.
Provide proper shoring
Conduct inspection to check the verticality/plumbness and squareness of the forms.
Check and review approved SRF for table forms and the location of permanent and
temporary shoring.
Establish reference gridline and elevation for the lay-out of girders, beams, columns,
shearwall, retaining wall and RC wall.
Double check the lay-out, elevation and dimension of girders, beams, columns,
shearwall, and retaining wall and RC wall before form installation.
Prior to setting of table forms, installed I-ribs on previous construction joints that may
contribute to honeycombs and gap between old and new concrete shall remove.
Set table forms as per approved shop drawings by the Structural Works Design
Consultant.
Coat the contact surfaces of the table forms with form oils. Do not use mineral oil on
forms of surfaces to which adhesive, paint, or other finished materials to be applied.
Check the elevation of table forms and beam bottoms.
Set forms mortar-tight and true to line and grade.
Provide proper shoring base on the approved SRF for temporary and permanent shoring
to avoid deflection.
Provide shear key and SONOBAND concrete epoxy in every construction joint. For
construction joint not at L/3, provide shear bars.
Provide trimmer bars on all block-outs and sleeves.
Chamfer above grade exposed joints, edges and external corners or concrete, 100mm
unless otherwise indicated or approved.
Provide clean-out openings especially on beams and girders to permit inspection and
removal of debris.
Camber all reinforced concrete beams only for every 10000mm of clear span.

When a beam crosses a girder, rest beam bars on top of the girder bars. At column
intersection girder bars shall be on top of beam bars.
After placing of concrete, forms shall remain for the following minimum time period, not
necessarily consecutive, where minimum temperatures specified in paragraph entitled
“Curing Period and Minimum Temperatures” are maintained adjacent to the concrete and
formworks. The minimum time period for removal of forms shall govern where it exceeds the
minimum specified curing period. Where the formwork for one element supports the formworks
for another element, the greater time period shall apply to both elements.
For all Vertical Structures, the minimum removal/stripping of forms are as follows:
For the removal of table forms, two-Strong floor shall be considered and maintained.
Forms may be removed earlier than specified if ASTM C 39 Test results of field-cured samples
from a representative portion of the structure indicate that the concrete has reached certain
design strength capable of supporting the induced construction loads for suspended structures.
Earliest will be 4 days where concrete testing is to be done on field-cured samples from a
representative portion of the structure as to validate attachment of 70% concrete design strength
and there exist adequate permanent or undisturbed shoring. Re-shoring has to be done after
removal of table forms and beam bottom forms.
Note: For the beam and girder soffit, there shall be permanent shoring installed at every
L/3 of a clear span. And for re-shoring, there shall be 2pcs of 30 tonner equally distributed
at every L/3 and middle third of a clear span to avoid deflection during curing of concrete.
11. INSPECTION
Conduct inspection after form removal to verify the squareness, plumbness, and
alignment of columns,beams, girders and correct elevation and thickness of slab.

12. RESPONSIBILITY
The QA/QC Head (NGCB) is the overall responsible body for the implementation and execution of
the works. At the same time, maintaining construction’s quality assurance and quality control
program and quality standards.
The Project Manager (NGCB) will be responsible for all construction activities in the site and will
directly supervise through Construction Supervisors, Foreman and skilled workers at the site. The
QA/QC Head will be responsible for arranging and controlling the inspection and testing activities.
Notwithstanding the above, it is the responsibility of each individual employed in the project to take
due care to ensure their own personal safety and that of their work colleagues.
