STATEMENT OF COMMITMENT

We MIGENLOGISTICS CORPORATION, owner of truck with plate number 1230-104015


and Foodlane Accreditation Certificate Control No. _____________ issued on ____________________ shall
strictly abide by the rules and regulations of the Foodlane Project;

We will comply to the rules and regulations and policies including all traffic rules;

We will report any misuse of this privilege by any parties hereto and;

We authorize DA to: verify and inspect my office, warehouse, truck and documents/records;
verify from whatever sources even after the Accreditation Certificate has been issued, the
veracity of the entries/statements in the application form and supporting documents as well as
the genuineness of the said documents; and to conduct visitorial activities during the life-span of
the Accreditation Certificate.

We understand that the accreditation will only serve a 24-hour passage at the identified
foodlane routes and can be revoked by DA for any violations thereof.

So therefore,We hereby bind or Corporation through my signature in this oath.
