I. RATIONALE:

The Home for the Elderly is presently upgrading its amenities and walkway aesthetic
design. To continue improving its facilities and future services to the public, a good
drainage system should be considered. The existing drainage system of Home for
the Elderly located at San Juan, La Union is no longer efficient because during
heavy rainfall, the water starts to overflow. Instead for the storm water to be drained
through the canal drainage, the water tends to build up quickly which causes flood.
This problem may bring hazard to clients and staffs upon entering and leaving the
area. The incompetent flow of water, the backflow from the main drainage source
and the capacity of the canal are the failures of the existing drainage system based
on the evaluation conducted by the Buildings and Grounds Maintenance Team. To
address the drainage problem, it is highly recommended to provide a good and
sufficient drainage design and maintenance work to prevent flooding which serves a
lifetime purpose to the building. Hence, this proposal is submitted.

II. OBJECTIVES:

The new drainage system is designed to provide its objectives: to prevent flood, to
secure long time service and to have efficient drainage system.

III. DESCRIPTION OF THE PROJECT:

The proposed project consists of construction of new drainage canal connecting to


the existing drainage system and improvement of walkway beside it. This new
proposed canal measures 60 meters long in total (left and right part of the building),
0.30 meter deep, and 0.30 meter wide. All sidewalls and floors of the proposed canal
shall be made of concrete and reinforced with Grade 33 steel bars. The drainage
canal shall have a steel grating covering following the steel frame design of the
existing canal. The project is estimated to be completed within 30 calendar days.
IV. BUDGET REQUIREMENT:

V. RECOMMENDATION:

The implementation of this project will improve the current situation of the Home for
the Elderly’s drainage system and will enhance the walkway aesthetic view hence,
this proposal is highly recommended for approval, and fund allocation will be
sourced out from the Program Management Bureau.

VI. RATIONALE:

The DSWD Field Office 1 is presently improving the electrical system design of the
building to provide comfortable and safe working environment. The building needs
good electrical design because it is very essential for the safety and health of DSWD
employees and clients. Therefore, we recommend that the electrical equipment to be
use must be in accordance with the provisions of the latest Electrical Code of the
Philippines to ensure quality of the materials. The Building will have a high quality
standard of electrical design and will solve issues and concerns, which are
associated to electrical design and development to ensure that they are economical,
safe and reliable. Hence, this proposal is submitted.

VII. OBJECTIVES:

The aim of this project is to provide convenience for DSWD workers and to utilize
electrical system for the safety of each individual.

VIII. DESCRIPTION OF THE PROJECT:

The DSWD maintenance team will perform the installation of additional electrical
outlets. The materials to be used should be in accordance with the required
specification of the National Electrical Code of the Philippines.

During Implementation, the Buildings and Grounds Management Section will do


monitoring and supervision. Daily monitoring will be made to ensure quality works
and reporting of accomplishments and issues will be done weekly in order to prevent
aggravation of issues and respond to the needs of the works immediately.

IX. BUDGET REQUIREMENT:

Electrical materials for the additional ACU outlet supply of DSWD FO1 3 rd floor
extension = Php 89,932.80

X. RECOMMENDATION:

To provide quality performance and safety of clients and staff, it is very important to
improve the electrical design of the building, it is also recommended to use quality
materials and equipment for the implementation of the said project to prevent fire
incidents hence, this proposal is recommended for approval, and fund allocation will
be sourced out from the Fund of the Field Office.

Project Title: LABOR AND MATERIALS FOR THE PROPOSED ACOUSTIC


INSULATION OF DSWD REGIONAL OFFICE BUILDING

XI. RATIONALE:
The DSWD Field Office 1 is presently improving and expanding the existing building
from two to three storey/floor area to have a better and convenient place to serve
clients. The Field Office 1 is composed of 7 (Seven) Divisions with an average of 16
staff per Division and catering to more than 100 clients a day. With this number noise
can interfere and cause distraction and discomfort particularly in areas were wood
flooring is installed. To minimize, if not eradicate noise from the third floor, this
project is proposed.

XII. OBJECTIVES:

The aim of this project is noise reduction or elimination from the third-floor level of
the building to the Second floor.

XIII. DESCRIPTION OF THE PROJECT:

The Acoustic Insulation will be installed at the third floor. The materials used in the
floor slab of the third floor - level are pure timber design and the ceiling of the second
floor is only on its standard height of 2.4m from the slab of the third floor to the
second-floor slab. During the construction, noise was obviously notice that causes
distraction and discomfort at the lower level of the building during operation of
offices. To reduce the sound coming from the upper level of the building and to allow
staff at the lower level to concentrate on their work, soundproofing material on the
timber slab of the third-floor level of the building will be installed. Specifications are
shown in the attached Detailed Estimates.

The procurement will be made through Negotiated Procurement – Small Value


Procurement of infrastructure project as indicated in the Supplemental Project
Procurement Management Plan for CY 2019 of the Administrative Division.

During implementation, monitoring and supervision will be done by the Buildings and
Grounds Management Section. Pre-construction meeting will be conducted with the
contractor once award has been received and acknowledged and contract has been
signed. Daily monitoring will be made to ensure quality works and reporting of
accomplishments and issues will be done weekly in order to prevent aggravation of
issues and respond to the needs of the works immediately.

XIV. BUDGET REQUIREMENT:

Labor and Materials for the proposed Acoustic Insulation = Php950,289.38

XV. RECOMMENDATION:

This project is seen vital in ensuring quality performance and wellness of clients and
staff. It is unforeseen during the implementation of the preceding projects hence, this
proposal is recommended for approval and fund allocation be sourced out from the
Fund of the Field Office.

Project Title: IMPROVEMENT OF DSWD FO 1 FRONT OFFICE BUILDING

Implementation Site: DSWD Field Office 1, City of San Fernando, La Union

I. RATIONALE

Improvement of the Field Office has been going on to ensure better services to the
clients being served and a more conducive workplace for the staff. The Front façade
has been improved according to the design and available funding at the time of
planning. However, finishing and partition works have not been completed due to
limited funding. Partitions are necessary to make sure that office spaces are
available for staff to facilitate service to clients. Finishing works need to be done to
provide corporate image that encourages clients to feel free to transact business with
the office and for staff to have a sense of pride in their work. Hence, this project is
proposed.

II. OBJECTIVES:

The project ensures installation of DSWD Signage to easily identify the office and to
comply with standard rudiments.

III. DESCRIPTION OF THE PROJECT:

Variations on the completed project have been proposed but due to limited funds at
that time and the cost of more than allowable extra works, these were disapproved
leaving the project substantially complete but lacking finishing works and partitions.
This project’s main scope of works includes installation of composite wall cladding,
standard stainless letters (DSWD signage) and canopy for the terrace at the upper
deck of the building. Specifications are shown in the attached Program of Works
along with the needed supporting documents.

V. RECOMMENDATION:

To maximize the use of the front office and ensure corporate identity, there is a need
to put signage and standard rudiments of the building. Office spaces need to be
partitioned to be utilized as offices and improve work space of staff for better
performance. Hence, it is highly recommended that this project be approved, and
funds be allocated for the purpose.

Project Title: Supply and Delivery of Materials for the Construction of


Basketball Court Cover at the Urayong, Bauang, La Union

Implementation Site: DSWD Regional Rehabilitation Center for Youth,


Urayong,
Bauang, La Union

VI. RATIONALE

The implementation of the Court Cover needs to be completed in accordance to set


standards. Works had been suspended because of unforeseen concerns that was
reported by the Contractor. The Regional Project Monitoring Team with the technical
assistance from the Central Office Engineers and the DPWH assessed the project
and agreed to strengthen it by replacing columns with bigger specifications.
Contractor agrees to provide the labor requirements due to high price increase. To
ensure completion and maximize utilization of the project, this proposal is submitted.

VII. OBJECTIVES:
The project seeks to comply with the agreements and recommendations of the
DPWH and ensure maximum utilization and to avoid possible structural issues that
may arise in the future.

VIII. DESCRIPTION OF THE PROJECT:

Materials that will be used for replacement of the installed angle bars will be
purchased by the Field Office while labor will be provided by the contractor of the
existing project. These materials will be delivered directly at the implementation site
in RRCY, Urayong, Bauang, La Union and will inspected onsite. Once complete
delivery has been made, construction works will resume.

IX. BUDGET REQUIREMENT:

X. RECOMMENDATION:

The project is aimed to provide RRCY clients with a venue to conduct recreational
activities and improved their well-being. If the project will not be completed, clients
right to recreational activities and good health will be deprived. Completion of this
project is vital hence, it is highly recommended that this proposal will be approved
and funds will be allocated for the purpose.

Project Title: LABOR AND MATERIALDS FOR THE PROPOSED DEEP WELL OF
THE FIELD OFFICE 1

Implementation Site: DSWD Field Office 1, City of San Fernando, La Union

XI. RATIONALE

The Field Office 1 at present is procuring potable water supply for use of the clients
and staff. Issues were encountered to include compliance of suppliers to the
provisions of the contract which resulted to contract termination. In the past, deep
well and water district subscription/connection were available however discontinued
due to no water supply.

However, the problem of inadequate water supply continued up to this time. Hence,
consideration of having a new source of potable water system is being
recommended by constructing a deep well inside the FO site.

XII. OBJECTIVES:

The project aims to provide a potable water system that can supply adequate water
for use of the clients and staff.

XIII. DESCRIPTION OF THE PROJECT:

The project aims to revive the existing deep well. Once water source is identified,
complete potable water system will be constructed connecting the water supply to
the reservoir. Water pipes will be installed towards the buildings to ensure access to
adequate potable water. Specifications are shown in the attached bill of quantities
along with other supporting documents.
XIV. BUDGET REQUIREMENT:

XV. RECOMMENDATION:

Water is important in day-to-day activities of the people in the Field Office. Issues
encountered could be addressed by having our own potable water system that could
adequately supply the needs of the office, its staff and the clients we serve. Hence, it
is recommended that this project be approved, and funds be allocated for the
purpose.
