2. Eligibility Criteria to avail the service: 

  1. He/She must be resident of Haryana State.  

3. Supporting Documents (scanned copy from original document to be uploaded): 

  1. ID Proof 

  2. Address Proof 

  3. Report of Patwari/ MC 

  4. Self attested declaration

5. Workflow Stages for delivery of services.  

  1. Applicant fills up the Application form. 

  2. Done verification from concerned Gram Sarpanch (Rural)/Ward Municipal Councilor (Urban).  

  3. Get verification from concerned area Patwari. 

  4. Applicant will visit to CSC Centre. 

  5. Document checked by concerned VLE/CSC operator. 

  6. VLE will fill up the CIDR details and generate CIDR ID. 

   7.  VLE operator scans the original documents and upload as single PDF file. 

  8.  The application along with attachment will electronically forward to the concerned authority   
(Tehsildar/Naib Tehsildar). 

  9. Transaction ID generated and issue acknowledgement slip to the applicant. 

  10. Tehsildar/Naib Tehsildar receives the application electronically in their user login. 

11. After scrutiny of scanned documents and take decision regarding the approval. 

12. On approval, the facsimile signature will get attached with the certificate. 

  13. VLE Operator/ Concerned Applicant can download the certificate by providing Transaction ID.  

   

6. Form Filling Instruction: 

1. All the parameters must be filled up properly by the applicant. 

2. Supporting document must be attached with the application form. 

3. Application must be duly verified by the concerned verifiers.
