Directions: In order to answer the questions below, review your Weekly Fitness Logs
since your last checkpoint. Don’t forget that your parent/guardian must also submit a
Fitness Log Verification Form in order to receive credit for your Weekly Fitness Logs.
1. Do you feel like you have increased, decreased, or maintained your fitness level over
the past few weeks? Explain.
decreased, I've been pretty busy because I started a new play and I have homework and
stuff so I haven't had a lot of time
2. What has been the most challenging thing related to your exercise routine in the last
few weeks?
trying to find time and motivation to go to the gym

weeks?
I feel good when I actaully get a good hard workout in

none really but I'll try to keep exercising.

Parent/Guardian Verification
In order to receive credit for all Weekly Fitness Logs, the parent or guardian of the
student must submit the following verification letter at regular checkpoints as
outlined in the course schedule. This verification should be submitted by email to
the instructor email provided at the beginning of this course.

submit verification for all weekly fitness logs since the previous checkpoint.
Consult the course schedule for further details.
